Our Physical Therapists see between 6-8 patients per day, with an average of 2 New Patients per day. We focus on the quality of treatment and care versus the quantity of patients in our office. Immunizations
We recommend all patients with a trained service animal carry the animal’s current veterinary health form. GPT reserves the right to request those documents at any time. Patients must have a current Veterinary Health Form and/or an immunization record on file or other proof of current vaccination.

Carpal Tunnel Syndrome
An Upper Quadrant/Carpal Tunnel Test is used to determine the capability of the prospective employee in relationship to repetitive job function, as opposed to testing physical strength. This test may be done in correlation with a Post-offer, Pre-Employment Test, the Fit-for Duty (for injured workers), or independently as its own stand alone test.

Mental Health
Complete the Emotional Support and Psychiatric Service Animal documentation before bringing the animal into the clinic. Currently, we require documentation that is no more than a year old from a certified mental health professional indicating need for an emotional support animal, plus certification of the animal’s health including rabies shot verification.

I’d like to express my sincere appreciation for your, Rhonda’s and Sarah’s care earlier this year… I went through approx. 3 weeks of physical therapy prior to my neurosurgery appointment. By the time of my appointment, I had experienced a significant reduction in pain and numbness. I continued physical therapy past the appointment… The 2nd neurosurgeon recommended that I continue PT, and I did so… Since PT, I’ve continued exercise and conditioning and I now consider myself to be 95%…[and] I am 100% grateful for the help, care and guidance and provided by you and your staff. I am appalled to think that if I had listened to the 1st neurosurgeon’s recommendation, I would have gone through major surgery (removal of 3 cervical discs and related reconstruction) and now be in the process of recovering from the surgery… THANK YOU, THANK YOU, THANK YOU!

Great place to go for PT. Everyone is so nice. Had a full knee replacement and they are helping so much to get mobility back. Would definitely recommend going here for PT.
