The campus includes Concord Reserve Assisted Living Community, Center for Advanced Rehabilitation and Lutheran Home Skilled Nursing and Long-Term Care. Concord Reserve?s Senior Life Community offers optimal health, healing, hospitality, happiness and hope for every resident. Concord Reserve Assisted Living offers secure Alzheimer's and dementia suites as well as care for physically frail seniors. Basic services include meals, housekeeping, activities and transportation. Individualized care plans including medication management and personal care are tailored to each residents needs. Center for Advanced Rehabilitation provides therapies for residents to achieve their highest level of independence. Optimum function and mobility can be achieved through physical, occupational, speech, massage and music therapies. For over 75 years, Lutheran Home at Concord Reserve has offered exceptional skilled nursing and long-term care services. Lutheran Home achieved a 100% deficiency free state survey in 2008. Our direct resident care exceeds state requirements and we boast a dedicated volunteer roster of over 100 strong.
