If you're a woman whose breasts have gradually sunk lower and lower over the years, don't feel that you shouldn't have gone braless in your younger days or that you shouldn't have jogged so much. Most people who achieve significant weight loss discover an unfortunate side effect in their now droopy breasts. And many women who have had children notice the deflating effect pregnancy and breastfeeding have on their curves.

Cosmetic surgeons or plastic surgeons typically perform breast implant surgery. Some plastic surgeons are cosmetic surgeons, but not all. Many cosmetic surgeons are not originally trained as plastic surgeons, and come from many surgical backgrounds, including otorhinolaryngology, general surgery, ophthalmology, and obstetrics and gynecology.
Cataracts
Correctly done tumescent microcannula liposuction with only local anesthesia (and no IV fluids given) is one of the safest cosmetic surgical procedures done in office surgical suites or ambulatory surgical centers. (Please see the above chart) Done in this manner, it is probably as safe as (or safer than) modern cataract surgery, particularly considering the fact that the average liposuction patient is much younger and healthier than most cataract patients. In addition, many cataract patients are given systemic anesthesia.
Cataract Surgery
The method of anesthesia and numbing have nothing to so with the amount and quality of the surgery done. They simply allow the patient to be comfortable while the procedure is done. Surgical techniques "evolve" and improve. Just as cataract surgery used to require a large incision with many sutures, and sandbags to the head for two weeks (as late as 1979), cosmetic surgery has changed. Very effective, pleasing cosmetic results do not require suffering. Many of the more traditional facelift techniques have not changed for 35 years. Liposuction techniques, invented by a French obstetrician around 1970, changed very slowly until 1987, when tumescent anesthesia was invented. Nineteen years later, most plastic surgery programs do not teach true tumescent anesthesia. In fact, the plastic surgeons in California continue to attempt to deny hospital tumescent liposuction privileges to the very dermatologist who invented it, contending that he is unqualified to perform the surgery which he invented, and taught all of them. (He is 57 years old, very active, and very healthy) Go figure. Clearly, patient comfort and well being is not paramount in these issues. Often, "less is more." Safe and comfortable surgery, a rapid and comfortable recovery, and an excellent result are obtainable goals, without suffering or pain. For instance, one of the myths about laser facial resurfacing is that "the longer the face is red, the better the result." This is nonsense, but it is nonsense spread by some surgeons who use very hot lasers and create months of redness. I have even attended a liposuction conference where a surgeon who does not perform true tumescent liposuction ridiculed this procedure, implying that there is something wrong with the patient being safe, awake, and comfortable. Wrong for whom? When that same surgeon's wife has liposuction, guess what? If he loves her, he'll want her to have true tumescent liposuction. Remember, it's not the surgeon who suffers from an outdated, less safe, less comfortable surgical method. You do. Last time we checked, the patient is paying the surgeon for a service. What patient would go to a surgeon and say, "You know what, I don't want the newest, most effective techniques, I'd like some good old fashioned 'retro surgery,' complete with more pain and suffering."

After initial enthusiasm for internal ultrasonic assisted liposuction, most European liposuction surgeons eventually rejected it because of an unacceptable incidence of complications. The European experience has shown that internal UAL is less safe than tumescent liposuction. No convincing data show that external ultrasound provides any clinical benefit. In spite of this, many American liposuction surgeons continue to use internal and external ultrasound for liposuction.

With its re-approval of silicone filling in 2006, the FDA included a recommendation that patients have MRI scans periodically. Should a shell rupture, the silicone filling is not likely to migrate outside the breast, thus the FDA has termed this possibility a "silent rupture." The FDA feels that women should be prepared for periodic exams to check for leaks.

At this point, the procedure has been supplemented by incisions that are more limited across the scalp. The multiple incision technique provides for faster recovery and reduces scarring. These techniques can be combined with the use of an endscope to assist with the surgical procedure. The recommended technique is individualized to the patient's anatomy and complaints. Adjunctive rejuvenation procedures such as lasers, chemical peels, and Botox can be combined with the operative procedure. The procedure takes about 1-2 hours of operative time and is performed in our on-site surgical operating room.

A brow lift can reduce the lines to the forehead and brow. The procedure minimizes or eliminates the creases across the forehead, and the "frown lines" above the nose. It is also possible to raise the eyebrows to a more youthful position.

In aging, the eyelid skin may sag or droop producing puffiness or bags. These conditions are corrected by eyelid surgery to regenerate and improve the appearance of the eyes.

Other occasional complications are common to almost any surgery. A few patients experience unusual swelling, and infection around the implant happens rarely as well. These conditions are almost always treated fairly easily. Occasionally, a patient will be unhappy with a surgical scar; when this happens scar revision can usually be performed in a short, in-patient appointment.

There are currently three major techniques available for buttock augmentation and body contouring. The three techniques are buttock augmentation with a silicone implant, buttock augmentation with use of an injectable implants e.g. PMMA, and the third and most popular technique is the use of one's own fat, usually called a "Brazilian Butt Lift".
Laser Resurfacing
For some patients, this can produce more pleasing results than a face lift, and may reduce or eliminate the need for a brow lift, eyelid surgery, Botox, or implantation of the cheeks, lips, and/or chin. Patients who may have thought of laser resurfacing for wrinkles or acne scars may be significantly improved, in a much different manner, by facial fat injections. This can also be combined with other procedures, for true, "multi-dimensional" facial rejuvenation.

Microdermabrasion is a non-painful, non-invasive method of improving the skin. It can be easily done in Dr. Sampson's office in Jackson, Michigan. In addition, can be easily done within a lunch- time.

Allergies
Tumescent anesthesia is given through areas that are first numbed with a very tiny needle, the size used in allergy testing. A dilute solution is then placed beneath the skin, a process which often takes 45-60 minutes for liposuction, 15 minutes for a facelift. The solution is then allowed to "diffuse" for 30 minutes before any surgery is begun, allowing widespread, deep numbing. (This is the step which is ignored by many physicians, since it requires 30 minutes of "downtime.") Some patients will talk, some will sleep while the procedure is done. More solution can be given at any time if you become uncomfortable. The most important advantage is that the anesthesia is not given in your veins, and does not affect your whole body. Patient acceptance is superb, comfort during surgery is usually excellent, and the recovery is many times easier than after being "put to sleep."
