Before my gastric bypass surgery in 2008, I wore a size 24, had sleep apnea, high blood pressure, GERD, depression fibromyalgia, Lyme disease and various other ailments too numerous to list…sigh I now wear a size 4, no sleep apnea, no high blood pressure, no GERD (thanks to Dr. Duperier who removed a hernia the size of a dinner plate that no one else seem to be able to find) and the depression has decreased.The Fibromyalgia and Lyme are still with me and probably always will be, but he told me from the start that he could not fix those. Dr. Duperier’s professionalism and caring bedside manner really made the whole process a wonderful experience. His staff was outstanding from the very beginning and still are some of the nicest medical personal and office staff I have ever had the pleasure of dealing with. Primary Care
The disease is often diagnosed after a primary care physician performs routine laboratory tests that indicate there may be an abnormality of the parathyroid gland hormones. One indicator is elevated calcium levels in the blood. In patients who are not hospitalized, elevated calcium levels generally indicate the need for further evaluation for hyperparathyroidism. The primary care physician generally refers the patient to an endocrinologist for further work-up and evaluation. If the biochemical tests performed confirm that the patient has parathyroid disease, the patient is referred to a surgeon for surgical removal of the parathyroid. Medical treatment is not ideal for parathyroid symptoms.

I have been struggling with my weight for over 20 years now; at the age of 42 I was dealing with High Blood Pressure, Sleep Disorder, Hurting joints, fatigue on top of being the heaviest I have ever been at approx. ~330lbs. I was of the mindset that I could do it on my own without any help. This resulted in failed diets, poor self-esteem and just more weight gain. It seemed like every time I dieted, I would lose 20lbs and gain back 30. It was a cycle that brought on depression to the point I did not like taking family photos or be in front of my own wife with my shirt off. I joked about my weight in front of friends before they got the chance, in my mind it seemed less painful but it wasn’t. I always wondered what they thought about me. I could not fit comfortably in an airplane seat. I always needed and extension for the seat belt. I could not stand and put my belt in my jeans because I lacked the flexibility to reach around and loop it through my pants. Honestly sitting here typing this up I can’t help but feel little humble and excited to share the next part of my journey with you. In August of 2014 I had my first consultation with Dr. Duperier, and I must say his straight forward attitude caught me off guard a little. His first real question (after the initial pleasantries) was what surgery are you looking at and why? At this point I was faced with a choice, to continue to mask how I really felt or come clean and get help. I decided to lay it all out on the table and see what his reaction was. And to my surprise He was very respectful and I could tell that he really enjoyed helping people change their lives. I remember him saying something like there is no magic pill and you are going to get out of it what you put into it. This is not going to be easy and I needed to prepare for a lifestyle change. And change is what I prepared for the next 12 months(Insurance Requirement). My surgery came on October the 9th 2015. I went in on Friday and had the gastric sleeve procedure and was released on Saturday to go home. I also took Monday off and was back to work fulltime on Tuesday. I began walking two miles a day as part of my recovery. Learning is a never ending cycle, My body is communicating with me now and I am listening. After nearly 90 days since my surgery I am happy to share that I am weighing in at 238lbs, dropped 5 pant sizes, not using a sleep apnea machine, and reduced the amount of blood pressure medicine I was taking. I have lost several inches around my neck, chest, waist and arms. I feel like a brand new person.(92lbs overall, 62lbs since I left the hospital) If you are reading this and thinking about taking the next step to change your life or just want more information, please do so, don’t wait. If you have a friend or family member that you are doing research for please pass this along. I am thankful every day that my wife kept pushing me to go in and visit with the doctor. That first visit changed my life!!!!!

A native of Houston, Texas, Dr. Jennifer Seger is a graduate of the University of Texas at Austin and the University of Texas Medical School at San Antonio. During medical school, Dr. Seger served as Class President for two years, was a Peer Advisor for incoming medical students, received the Paul J. Hi, my name is Andrea Farmer. I don’t always know where to begin with my story. Some people want to hear about my weight loss while others want to hear about regaining my life and being free from pain. So, I’ll start at the beginning and hope my story inspires some to have hope and faith. I had just moved back to Tulsa, Oklahoma because my husband was going to Korea for a one year tour. We had two small children at the time. Luke was turning 3 and Mitchell was about 4 months old. I had been having headaches and thought it was simply having two kids and a husband. (LOL) Then I woke-up one day with my eyes crossed. I thought, well this is just what I need, crossed eyes to go along with the headaches. My husband, John got me right into a long time family doctor who knew right away this was serious. Of course, I shrugged it off as stress. Nevertheless, I was sent straight to the hospital. I was later diagnosed with Pseudo Tumor Cerebri. Basically means “false tumor of the brain”. My four ventricles were no longer moving the spinal fluid around in my brain and it was not being absorbed into my body. Therefore, the pressure would build in my head and cause the headaches and my eyes to cross. Lovely, huh? Well, my husband being the military hard-core guy that he is was simply beside himself. He still went to Korea. I went on to try numerous drugs and eye surgery before I had brain surgery to drain the fluid off of my brain and insert a shunt in my spine. It was a terrible recovery. But by the time John returned from Korea, I was up and walking again. Couldn’t hardly tell I was sick. Other than the terrible headaches, nausea, stars, and dizzy spells….I was fine. I continued to do the work, be a mom of two and a wife. So, lets fast forward a couple of years…I started to feel even worse. I returned to my neurosurgeon and he said I now had hydrocephalus in addition to the Pseudo Tumor Cerebri. So I had another brain surgery to insert another shunt. This time in my brain. I never did feel any better. I either had too much fluid in my head or not enough. I simply would get through the day the best I could. I continued to move all over the country as a good military wife and saw numerous neurosurgeons who never wanted to treat me. The depression set in and I turned to food for comfort. I wasn’t moving around anymore either because the more I moved the worse my head felt. Well, the weight came on slowly at first, but before I knew it I was a size 24 and 254 lbs. I had another brain surgery to remove one shunt. I kept the other one so my brain wouldn’t hemorrhage. Life was crazy…I developed high blood pressure and was now packing my head in ice most every night to get some rest from the pain. I had been suffering for 10 years at this point. Endometriosis
Gastric Bypass surgery was a decision that was thought of many years ago. Never felt that I could or wanted to. I had gone to Dr. Seger’s seminar way before at the Medical Towers that my PCP recommended that I go to. His demeanor was like no other Dr. Did I decide that day to do it? No actually I didn’t. Years went by I found a new primary Doctor. Dr. Linda Esquivel also brought up gastric bypass surgery. She didn’t push me to have it, it was just mentioned. Even my OB/GYN brought it up because I have endometriosis. She couldn’t perform a hysterectomy due to my weight. I even couldn’t be put to sleep due to my weight. Cardiologist never pushed me. Dr. Ortiz felt if it was going to happen, it’s going to happen. I was prescribed 14 different medications. I would take 2 oral meds for diabetes. 1 shot of Lantus of 80 units a day. 20 mg of Humalog 3 times a day. That was only for diabetes So here comes August 29, 2011 when the decision was made to have the surgery finally, I remembered Dr. Seger. He was the one I knew that was going to take care of me. So in August of 2011 I finally got the courage to go speak to the staff of BMI of Texas. This is a place that performs Gastric Bypass, Gastric Sleeve & Lap band surgeries. At that point the process began. Dr. Seger saved my life. The BMI of Texas staff has saved my life. I have no regrets of having this surgery. I would do it all over again. His goal was at 180 pounds. My personal was 190 pounds. I workout 6 days a week, I started walking at 10 minutes a day. Moved up to 20 minutes a day of walking, finally the most I have ever walked is 5 miles. Then finally I started taking classes at Gold’s Gym. I now take Body Combat, Body Pump & Body Attack. My instructors are amazing there. They gave me the confidence and the drive. I shadowed my first class with my Body Attack instructor. Body Combat is my heart and one day I hope to shadow one song. Until then I will keep moving and doing what is good for my body & heart. I am no longer on any medications for diabetes, high blood pressure or high cholesterol. I will be doing my first half marathon in December with the BMI of Texas staff at the Rock-N-Roll Marathon here in San Antonio. I have a huge family system and friend support that has guided me through this journey. I am so grateful because of this surgery. I will live longer for my Daughter who is 11 years old and is Autistic. She needs me so much but honestly, I need her so much. Without you BMI of Texas I would have never reached where I am now. You guys have given me the goals and strength to move on in my life. I been a prisoner in my body and now I know what it is to live for me. My daughter taught me life has so much meaning. But now, I know what it’s like to love myself and be there for me. Thank you guys with all my heart. I love you all with everything I am. I AM FREE!!! Many patients have a history of kidney stones or blood in their urine, or a recent history of bone fractures. Other symptoms include frequent urination and urination at night.
Endoscopy
Over time chronic acid reflux can lead to changes in the cells that line the esophagus. This is known as Barrett’s Esophagus. Patients can also develp pre-cancerous changes (dysplasia) as a result of chronic reflux. Once a patient develops these findings, careful and close follow-up is mandatory with an annual upper endoscopy to rule out the development of cancer.

Amber graduated with her Associates of Applied Science in Nursing from San Antonio College in 2008 and earned a Bachelor of Science in Nursing from University of Texas at Arlington in 2014. She began her career at one of the top hospitals in San Antonio, Methodist Specialty and Transplant Hospital. Initially, she cared for patients on the surgical units and in less than two years, she was promoted to charge nurse. In a pursuit for self-improvement, she moved to the operating room, where she mastered the nursing aspects of transplant surgery along with a variety of other surgery types. While robotic surgery was still in its infancy she chose to move into this growing specialty. She continued to excel and was asked to head the robotic program. A short time later, Huebner Ambulatory Surgical Center was being opened, and she was the first choice to be hired to develop and manage the bariatric, orthopedic, and general surgery programs.

She has focused her practice in obesity medicine, bariatric and general surgery. Some of her previous work experience includes working in Houston, Texas with her practice portrayed on Discovery Health television show, Big Medicine. She has worked for many years with BMI of Texas as well as other weight loss programs in San Antonio including the program at The Methodist Weight Loss Center. She was also the Clinical Director of a diabetes and wound care clinic in San Antonio, helping patients manage their wounds, nutrition and health. Minimally Invasive Surgery
A laparoscopic adjustable gastric banding (LAGB), aka LAP-BAND, is a minimally-invasive surgery that takes 1-2 hours to perform. It’s done through small incisions in the abdomen, through which thin tubes with cameras and tiny surgical instruments can proceed into the body. Using the instruments, a surgeon places a silicone gastric band around the upper portion of your stomach. A balloon-like structure rests on the inside of that band.

Dr. Englehardt specializes in weight loss surgery and has extensive knowledge on the surgical treatment of obesity. He offers patients a wide variety of surgical weight loss options, including Roux-en-Y gastric bypass, sleeve gastrectomy and adjustable gastric banding. The LAP-BAND® is an FDA-approved surgical device from the company ALLERGAN, a well-established pharmaceutical company in Ireland that has brought us many common drugs like Restasis, Latisse, Botox, Lexapro and Juvederm. A LAP-BAND procedure may be done with the Allergan product, but since it’s been around for some time, there are now other reliable generic versions of it.

Dr. Seger has presented articles at national and international meetings on bariatric and laparoscopic surgery. Dr. Seger is a past president of the Texas Association of Bariatric Surgeons. He has served as the chief of surgery at the Methodist specialty and transplant hospital in San Antonio. He chairs the Access to Care committee for the American Society for Metabolic and Bariatric Surgery. He is a member of the Society of American Gastrointestinal Endoscopic Surgeons, Bexar County Medical Society, and the American Medical Association. He is a Diplomat of the American Board of Surgery and a Fellow of the American College of Surgeons. He has earned a focus practice designation in Bariatric surgery from the American Board of Surgery.
Bariatric Surgery
BMI is proud to have one of the only advanced accredited minimally invasive Bariatric Surgery fellowships in South Texas. Surgeons from all over the country apply and compete for the opportunity to train at BMI of Texas. Our Fellowship is affiliated with the University of Texas Health Sciences Center at Houston and part of the Minimally Invasive Surgeons of Texas (MIST) program. Drs. Duperier and Englehardt serve as co-program directors of our fellowship which was started in 2006.

Lesions
Results of biopsies come in several different ‘flavors.’ Benign lesions usually require follow-up on a routine basis with yearly biopsies done by your endocrinologist. If a Fine Needle Aspiration result is malignant, we usually recommend a complete removal of the thyroid. This is called a Total Thyroidectomy. Occasionally, biopsy results come back as ‘suspicious.’ When a lesion is suspicious, it is not for certain whether or not this is malignancy and usually further workup is recommended. This comes in the form of having a repeat Fine Needle Aspiration done by your endocrinologist and/or it could mean surgical excision of part of the thyroid gland to obtain the ‘ultimate biopsy.’ In these instances, the pathologist ultimately becomes the one who determines whether or not further treatment would then be required.
