Primary Care
When you are living with a chronic disease or condition, you deserve a primary care provider who understands your condition and works with you to decide upon the best treatment plan. Dr. Amin and our providers coordinate with other specialists involved in your care to do what is best for you and your overall health. We believe in the holistic approach to patientcentered care, taking into account mental and social factors which effect us all, as well as helping you make lifestyle changes to remain as healthy and active as possible.
Family Practice
Piedmont Family Medicine is comprised of a specialized team of healthcare advocates referred to as an Advanced Care Team. Our Advanced Care Team members include a Nutritionist/Dietician as well as several Care Coordinators/Medical Assistants. Tanya Upton is our Nutritionist and she is available for consultations upon request or recommendation from our healthcare providers to provide education and counseling on topics such as obesity and diabetes. Meredith Sokolowski, Johnathon Boles and Ashley Wagner are our active Care Coordinators/MAs who oversee coordination of patient care to ensure that our patients are receiving optimal care. Our Advanced Care Team members play a vital role in closing gaps in patient care and in improving the level of quality care we provide.
Emergency Care
We also have a Co-Management agreement in place with Novant Health Urgent Care & Occupational Medicine located at 1904 Jake Alexander Blvd Ste 301 Salisbury, NC 28147. Agreement is in place to ensure continuity of care and to meet all of your Urgent care needs. They are open M-F 7:30am-7:30pm, Sat 8am-6pm and Sun 8am-4pm and can be reached at (704)638-1551.
