The risk factors for both central retinal vein occlusion or branch vein occlusion are similar and include high blood pressure, diabetes, cardiovascular disease, and glaucoma. If no predisposing risk factors are identified, a complete physical exam with laboratory examinations may be recommended. Some younger patients may need additional work up for blood clotting disorders.
Ophthalmology
Dr. Djafari graduated from Dartmouth College in 1992 with a B.A. in Biochemistry and Psychology. He earned his medical degree from Hahnemann Medical School in Philadelphia. Dr. Djafari then completed his internship at Brackenridge Hospital in Austin, Texas, followed by residency training in ophthalmology at Tulane University in New Orleans. Following residency training, Dr. Djafari undertook a fellowship program in vitreoretinal surgery at The Foundation for Retinal Research in New Orleans, headed by Dr. Kurt Gitter. Following felowship, Dr. Djafari took a position at The National Retina Institute outside of Washington D.C.
Macular Degeneration
Age-related macular degeneration (AMD) is a very common condition that affects the very center of the retina, an area called the macula. This tiny area is responsible for our central vision and allows us to read, perform most everyday tasks and recognize faces. Any problem with the macula results in a significant change of vision. AMD is the leading cause of visual impairment in people over the age of 65.
Laser Eye Surgery
Laser treatment for disorders of the retina is unrelated to laser eye surgery for vision correction. Treatment generally does not result in a change in your glasses prescription.
Cataracts
The retina is a very thin layer of highly specialized tissue that lines the inside of the eye. It is only slightly thicker than a human hair. The retina is analogous to the film in a camera. Light is focused on the retina by the natural lens of the eye (or by an artificial lens in the case of one who has undergone cataract surgery). The image that is focused on to the retina is converted to a chemical and electrical signal. This signal is preprocessed by specialized cells within the retina itself, and is then transmitted to the brain for interpretation. This process is so complex and energy intensive that the retina is the most metabolically active tissue in the body. The retina is similar to brain tissue in that it does not heal or regenerate spontaneously if it suffers injury.
Cataract Surgery
Surgery for conditions involving the retina and vitreous has seen tremendous advancements over the past 30 years. Some conditions can now be treated using in-office procedures, but more serious conditions often still require traditional surgery performed in a hospital operating room setting. Vitreoretinal surgery is often somewhat more complex and time-consuming than other eye surgeries. The retina is an extremely thin layer of tissue and requires very delicate maneuvers for successful surgical repair. We use a high-powered microscope in order to be able to successfully manipulate the structures within the eye, and even the tiniest patient movements can make the surgery more difficult. For this reason and due to the longer duration of surgery, we often recommend surgery under general anesthesia. Unlike cataract surgery, in which patients often note a dramatic improvement in vision immediately, the visual recovery from vitreoretinal procedures is often much more prolonged, and may actually be worse than prior to surgery for several days to weeks postoperatively. Additionally, it is sometimes necessary that the patient maintain a specific head position postoperatively to ensure the best possible surgical outcome. All of this can be daunting to a patient, but with attention to detail and appropriate discussion of surgical expectations, the process can be greatly facilitated and patients tend to do extremely well.
Eye Exam
Most epiretinal membranes are clinically insignificant and produce no symptoms or very mild visual disturbances. These membranes are often discovered on routine eye examination and present with a glistening or subtle sheen on the retinal surface. No treatment other than periodic monitoring is required, as approximately 70% of these will remain stable without intervention.

Epiretinal membranes typically occurs in those over age 50, and there is a slightly higher incidence in females. In most cases, the cause is unknown but predisposing conditions include retinal vascular disease (e.g. diabetic retinopathy), retinal tear, prior retinal detachment, and uveitis (ocular inflammation). The condition is mild in most cases and affects both eyes in approximately 25% of cases.
