Untreated chronic sinusitis can lead to fungal infections as well as damage to the sinuses, cheekbones or surrounding structures. Using a small camera called an endoscope, our surgeons can look up into the nose and operate on the sinus cavities and delicate intranasal anatomy without making incisions on the external aspect of the face. Your surgeon can then open the sinus cavities to improve drainage, reduce pressure and other symptoms while at the same time removing nasal polyps or abnormal tissue.

Medical Weight Loss
Treatment Once you have been diagnosed with OSA, treatment is usually recommended. The gold standard for treatment of OSA is positive airway pressure (PAP) therapy. This is a common treatment option that is prescribed by your PCP or Sleep Medicine doctor. Other medical options include weight loss with dietary and lifestyle modifications, positional therapy, oral appliances, and other devices such as nasal strips, chin straps, and more, depending on your anatomy and nature of your OSA.

Pediatric obstructive sleep apnea is a sleep disorder in which your child's breathing is partially or completely blocked repeatedly during sleep. The condition is due to narrowing or blockage of the upper airway during sleep.

Kidney Stones
The thyroid gland is located in the middle of the lower neck, below the larynx (voice box) and wraps around the front half of the trachea (windpipe). It is shaped like a butterfly, just above the collarbones, having two halves (lobes) which are joined by a small tissue bar (isthmus). You can't always feel a normal thyroid gland. Parathyroid glands are four small glands of the endocrine system which regulate the calcium in our bodies. Parathyroid glands are located in the neck behind the thyroid where they continuously monitor and regulate blood calcium levels through the parathyroid hormone, PTH. Unregulated growth of a parathyroid gland leads to elevated PTH, and elevated calcium levels on blood testing. Hypercalcemia causes many symptoms such as kidney stones, bone pain, chronic headaches, fatigue, osteoporosis, among others. Identifying and surgical removal of the abnormal parathyroid gland tumor is curative for the symptomatic patient.

An Ear, Nose and Throat doctor can perform a full head and neck examination as well as a specialized camera exam within your nose called nasal endoscopy.
Lung Cancer
Hoarseness lasting over two weeks can be a sign of cancer at the larynx or throat (pharynx). In the US laryngeal cancers account for 12,500 new cases annually, and less than 4000 cases annually for pharyngeal cancers. In contrast lung cancer accounts for 235,000 cases annually. Risk factors associated with developing cancers at the larynx and pharynx include smoking (current or former), vaping, advanced age, GERD, not vaccinated against HPV (human papilloma virus), strong family history of cancers. When detected early, cancers at these sites are successfully treated with surgery, radiation or combination of both modalities.

To be appropriately diagnosed and treated, a full head and neck exam by an Otolaryngologist is important. Some smell and taste disorders do not have any exam findings and further testing such as imaging (CT or MRI), blood work, and smell tests may help.
Radiology
Additional work-up for patients with chronic sinusitis may include a computerized tomography (CT or CAT) scan of your sinus cavities. This provides our surgeons detailed imaging of the internal component of your sinonasal cavities and may indicate anatomical obstructions, nasal polyps that could not have been detected on in-office examination. Our office specifically requests CT scans from our local radiology departments that are compatible with our intraoperative image guidance technology that allows our surgeons to identify an anatomical location with an accuracy of within 2mm.
Computed Tomography
The decision to proceed with surgery is based on your history and physical exam. It is important to bring any prior records with regards to your sinuses to your initial consultation. A CT scan of the sinuses is required preoperatively to accurately assess the areas involved. Surgery is for patients who suffer from chronic or frequent sinus infections when medical therapy has failed.
Chemotherapy
If you suffer from any of these symptoms, you should contact our office immediately and schedule an exam. Further screening tests may be prescribed based upon the type of cancer suspected. The most common treatment options for head and neck cancer is surgery, radiation treatment and chemotherapy.
Radiation Therapy
Malignant Tumors Malignant (cancerous) tumors can also arise from salivary gland tissue. There is a wide range of presentation of cancers in the salivary glands from indolent slow painless enlargement to rapid painful invasion of adjacent structures. In addition, cancers from other organ systems such as skin cancers or lymphoma can spread (metastasize) to the parotid gland. Tissue biopsy obtained through fine needle aspiration can determine the type of salivary gland cancer, which will dictate additional tests needed to assess the extent of the cancer. Personalized treatment of a salivary gland cancer is developed in collaboration with other specialists including radiation oncologist, pathologist, medical oncologist, neuroradiologist. Low grade salivary cancers are curatively treated with surgery. In contrast, after surgery to remove large, aggressive tumors additional treatment such as radiation therapy is administered to reduce risk of cancer recurrence.

Minimally Invasive Surgery
Transoral robotic surgery (TORS) is a minimally invasive surgery with the use of robotic technology to access areas in the head and neck that were not previously easily accessible. This includes the tonsils, base of tongue, and larynx (voice box). TORS is commonly used for cancers or tumors in these areas. Robotic surgery allows surgeons to have a high resolution, 3-D image with fine and precise movements to be able to treat patients more safely and effectively.
Robotic Surgery
HPV-related head and neck cancer cases have been increasing over the past decade. With more research in HPV related cancers and with the advent of transoral robotic surgery, there has been a paradigm shift in treatment. Previously, surgeries in the mouth or throat were 8-12 hour long with large incisions involving the lip, tongue, jaw, and neck. Many patients were hospitalized for several weeks with significant side effects including poor wound healing, difficulty with swallowing and speech, and need for possibly a permanent tracheostomy or feeding tube. With robotic surgery, surgery duration is typically 2-3 hours with shorter hospital stays, no scars, and easier recovery. Also, with the use of TORS, there is ongoing evidence that it can decrease the potential need for additional therapy after surgery for early stage cancers, and may reduce long term side effects from additional therapy for advanced stage cancers.

Blepharoplasty
Reshaping and augmenting the face and neck region can also be a safe and effective way to provide a desired aesthetic look. It is also sometimes required for reconstructive purposes due to skin or soft tissue defects from cancer excisions. Reshaping can include removal of excess fullness or skin but it can also mean contouring or repositioning tissue or structural components of the face. For example, blepharoplasty (removal of excess eyelid skin and/or repositioning of underlying fat pads) can help improve the appearance of the eye region so that a patient looks less tired and aged, and more youthful and energized. Some procedures can be completed in the office while others require surgery in a hospital or surgery center location. Craniofacial asymmetries, and bony prominences can also be addressed for facial harmonization, feminization and other desired aesthetic goals.

Skin Cancer
Skin cancers are the most common cancers in the United States and frequently arise in the head and neck region. Risk factors for skin cancer include sun exposure (UV light), tanning bed use, immunosuppressive medications (commonly used by transplant patients), and prior radiation exposure for treatment of other cancers.

Allergy skin tests are safe for adults and children, including infants. In certain circumstances such as having a severe allergic or anaphylactic reaction, medications that may interfere with test results, or if you have certain skin conditions (eczema or dermatographism), blood tests can be a more useful tool.

Benign Tumors Many types of benign tumors can occur in the salivary glands. The most common is pleomorphic adenomas (also called benign mixed tumor). Saliva cysts, saliva stones, reactive lymph nodes, Warthinâ€™s Tumor, oncocytomas, adenomas are other benign lesions to affect salivary glands. A full evaluation including a needle biopsy will inform the discussion with your otolaryngologist as to what treatment is in your best interest - to undergo surgical removal or to monitor.

Sports Medicine
The causes of nosebleeds are many. Nasal trauma, nasal inflammation, coagulation or blood clot forming problems, high blood pressure and nasal tumors are the most common problems. Bleeding from nasal trauma can be related to sports injuries, surgical procedures and foreign bodies inserted by children. Nasal inflammation can occur due to colds or viruses, bacterial infections of the nose or sinuses, nasal allergies, and irritation of the nasal lining such as from cigarette smoke or nasal steroid sprays. Coagulation problems can occur due to kidney or liver disease but are most often due to anticoagulation drugs such as aspirin, coumadin and newer generation drugs such as Xarelto. Poorly controledl high blood pressure can increase the risk of nosebleeds, and require input from a patientâ€™s primary care physician. Nasal tumors are a rare cause of nosebleeds and are found on nasal examination and/or nasal endoscopy. Episodes of bleeding increase during the winter months when the indoor and outdoor humidity is low, and the nose can become excessively dry.

Allergy treatment has a wide range of approaches. Environmental controls (e.g. HVAC filters, HEPA filters, household mold remediation), nasal rinses or targeted immunotherapy based on specific allergy testing are all options.
