Also, we specialize on post-hospital discharge follow-ups (Transitional Care) and on the uninsured. Our Goal is to prevent patients hospital's readmission and assist our patients to achieve their highest possible functional level, independence, and quality of life.Our holistic approach involves an Interdisciplinary Care Team approach composed by Nationally Board Certified Nurse Practitioners, Home Health Care Agencies, and local Community Resources.Our Team focus on disease prevention based on early detection and treatment, health maintenance and optimization by assisting our patients to identify and achieve needed lifestyle modifications.
