Ocular Surface Disease: These diseases are generally caused by problems with systemic inflammation, medications, previous ocular surgeries and intervention treatments. This can result from ocular chronic graft versus host disease (GVHD), Sjogrenâ€™s syndrome, rheumatoid arthritis, thyroid eye disease and any dry eye induced by other autoimmune diseases which effect the lubrication of the eye.

Outside work: Permitted. Fellows are encouraged to work in a corporate setting on weekends to maintain primary care clinical skills. Note, liability insurance and licensing will not cover outside work and priority must be given to call dues at CLEI.

Cataracts
A cataract is a clouding of the natural lens in your eye as you age. The natural crystalline lens is normally clear. As a cataract develops, the lens becomes more cloudy, yellow, and opacified, blocking light and a clear image from entering the eye. A cataract is painless and develops over time, causing progressive loss of vision.

Cataract surgery is generally an elective procedure. If your vision is good enough to drive safely, read, and perform your normal day-to-day activities, surgery may not be needed right away.
Eye Exam
Yes, PRK can correct astigmatism. It is important to note that not all cases of astigmatism can be corrected with PRK, and the success of the procedure depends on various factors such as the severity and type of astigmatism, the thickness of the cornea, and the overall health of the eyes. At CLEI, we will perform a comprehensive eye exam, with the most advanced equipment to help determine if PRK is the right option for you.

Dr. Greenstein received his medical degree from the Albert Einstein College of of Medicine where he graduated with a special distinction in clinical research. Dr. Greenstein completed an Internal Medicine Internship, prior to the start of ophthalmology residency training at Rutgers – New Jersey Medical School. Prior to and during his professional ophthalmic career, Dr. Greenstein worked closely with Dr. Hersh and the CLEI Center for Keratoconus in our research department.Â Dr. Greenstein completed a one-year pre-residency fellowship with us, where he concentrated his research on keratoconus and corneal cross-linking.Â Together, Dr. Greenstein and Dr. Hersh published a multitude of articles in prestigious medical journals, includingÂ CorneaÂ andÂ Journal of Cataract and Refractive Surgery, on CLEIâ€™s corneal collagen crosslinking (CXL) clinical trial for the treatment of keratoconus and corneal ectasia. Methods: In a prospective, controlled clinical trial, 21 eyes underwent CXL for KC or ectasia (14 and 7 eyes respectively). CXL was performed using the UVX system (Peschke Meditrade GmbH). All eyes underwent pre-treatment with Riboflavin 0.1% in 20% dextran solution every 2 minutes for 30 minutes. If the cornea was <400 microns thick, then hypotonic riboflavin was administered until the cornea swelled beyond 400 microns. Eyes were then randomized to receive either Riboflavin 0.1%/dextran (RIBO) or hypotonic (HYPO) riboflavin every 2 minutes for the 30 minute duration of UV exposure (UVA 365nm light for 30 minutes at an irradiance of 3mW/cm2). Corneal thickness was measured using ultrasound pachymetry (Sonogage).

Exposure Keratitis: This type of ocular surface disease is the result of impaired blink functions and incomplete closure of an eye. The exposed tissue experiences severe localized dryness which leads to the breakdown of the cornea and ocular surface tissue. This can happen after certain surgeries (after blepharoplasty or lid repair), lagophthalmos, anatomic lid malposition, paralytic exposure (after stroke or intracranial surgery), or acoustic neuroma.
