Sinusitis
Sinusitis refers to inflammation, swelling and/or infection of the sinuses. It can be acute, in which the sinuses fill up with pus and the child will have cold-like symptoms for more than ten days. These children may also have facial or tooth pain, although this is more common in adults. Sinusitis can also be chronic, in which the lining of the sinuses thickens and obstructing tissue known as polyps may develop. Chronic sinusitis may have an infectious component with bacteria in the sinuses, but it can also be present without true bacterial infection. These children usually have symptoms of nasal obstruction and drainage lasting several months. The drainage can either come out of the front of the nose (a "runny nose", also known as rhinorrhea ) or out of the back of the nose into the throat ("post nasal drip").

The audiology department in my office is staffed by licensed, certified audiologists who are dedicated to providing unsurpassed patient care in a relaxed, caring atmosphere. While we focus on pediatrics, we provide hearing services for adults as well. We can also do the initial followup testing for infants who may have failed their standard newborn hearing screening.

Ear Infections
Swimmer’s ear (otitis externa) is a localized infection of the skin of the ear canal. It generally has nothing to do with the types of ear infections young children get (which are behind the eardrum, and treated with antibiotics). It has more in common with diaper rash or athlete’s foot, which occurs in a warm, moist area of the body that are difficult to keep clean and dry. You don’t need to swim to get otitis externa; it can happen from excessive sweating, drainage from a middle ear infection, or even from attempts to clean the ear using Q-tips or other instruments.

Emergency Care
Postoperative bleeding from the tonsillectomy site often stops by itself. However, I always recommend that any actively bleeding child be brought to the emergency room for evaluation just to be safe. If the bleeding persists, it is usually managed in small children by returning them to the operating room for a brief procedure under anesthesia, in which the open blood vessel is found and cauterized. Some adolescents and adults may allow this to be done without anesthesia, but most children will not permit a thorough examination and cautery while awake.

When done early, babies rarely have any significant pain, beyond the few seconds that the procedure actually takes. Their discomfort is no more than they might exhibit with a vaccination, a diaper change or taking a blood sample. Years ago, I would inject local anesthesia into the area and it seemed to make their pain much worse, lasting for much longer.

MRI
Children with chronic symptoms of sinusitis (lasting more than a few months) are usually treated with a variety of medications as outlined below. If medical management is unsuccessful in relieving symptoms, a CAT scan may be useful if surgery is being considered. This scan will accurately show the anatomy and any inflammmation in the sinus cavities. Although an MRI scan can also sometimes show sinusitis (and does not involve x-rays), it is not as good for planning sinus drainage procedures since it does not show bone detail well.

This can actually be more dangerous than standard anesthesia. For some situations (such as painless but frightening procedures like a CT scan), it can be useful. But in a child with a small airway, the chance of breathing problems is greater if the airway isn’t under the anesthesiologist’s continual control.

Sometimes parents are told to wait to see their ENT doc until the swelling has gone down, but they aren’t given more information than that, so they show up 3-4 weeks after the injury, By this time the bones have healed in their displaced position, and it’s too late to do a simple closed reduction. All that can be done at that point is a “nose job” (also known as a rhinoplasty), where the bones are rebroken and then set into position. This is a much more complicated procedure, and generally isn’t done until after age 16.

Fungus
One very effective and safe treatment for both acute and chronic sinusitis is the use of humidification and nasal irrigation. Keeping the lining of the nose moist is important to the body's own natural defenses. Moisture also keeps the nasal secretions from drying out and blocking the natural sinus drainage pathways. Room humidifiers are helpful, but care must be taken to keep them clean, or they can grow colonies of bacteria and/or fungus. Nasal saline spray should be used frequently as well.

Finally, there is the condition known as Mal de Debarquement ("the sickness of getting off a boat"). Some people are unsteady and have difficulty walking after reaching dry land. This is thought to be an issue with the brain's adaptation to a new environment, and not an ear problem. In such cases, inner ear testing is normal, but the symptoms may last for a long time (months). Treatment with vestibular suppressants (like Bonine or Dramamine) rarely works, but sedatives and specialized physical therapy (vestibular rehabilitation) may be helpful.
