Glaucoma
If the lens capsule fractures during surgery, the natural lens may drop into the vitreous cavity. A dropped or retained lens causes inflammation and glaucoma and may require a vitrectomy to restore ocular health.
Ophthalmology
Nothing happens without disability insurance. When asking for a bank loan, the financiers will demand proof that loan can be paid, even if you become disabled. Group disability can be had through the American Academy of Ophthalmology (Marsh Insurance Co.) and private vendors. For an individual, more than one disability policy may be needed to carry the business overhead, personal and household expenses. Expect the application and approval period to take several months. So get the process moving early.
Macular Degeneration
Patients with a documented history of non- exudative macular degeneration with new-onset metamorphopsia and/or scotomata or a clinical exam demonstrating subretinal or intraretinal fluid or hemorrhage suggestive of a choroidal neovascular membrane should be referred urgently to a retina specialist for evaluation with fluorescein angiography, optical coherence tomography and initiation of intravitreal anti-VEGF therapy (Avastin or Lucentis). These patients need to establish care with a retina specialist, as the initial follow-up of the clinical response to anti-VEGF therapy is initially quite frequent (every four to six weeks).
Laser Eye Surgery
The onset of infection after eye surgery peaks at 72 hours after surgery – but may still arise even after several weeks (especially with suture-less surgery or surgical complications)
Cataracts
The overwhelming majority of procedures to remove a cataract and insert an intraocular lens go without complication. When a plastic or silicone lens comes in contact with the internal surface of the cornea, the corneal endothelial layer is injured. With excessive damage, at the time of surgery or post-implantation, the cornea may fail (corneal decompensation) to maintain sufficient transparency.
Cataract Surgery
Disruption of the anterior vitreous face “vitreous loss” predisposes the eye to develop cystoid macular edema and retinal detachment, especially if vitreous strands adhere to the iris and wound. Insertion of the intraocular lens into the pre-existing lens capsular bag is preferred.

Ultrasound
Any patient who has sustained penetrating globe trauma must be carefully evaluated for the presence of an intraocular foreign body. Several modalities are useful in assessing for a posterior segment intraocular foreign body, namely B-scan ultrasound, frontal and lateral skull plain films and non-contrast enhanced orbital CT scan. Foreign bodies may be metallic, glass or organic material. If an intraocular foreign body is suspected, urgent referral must be made to a retina specialist for removal. Pars plana vitrectomy provides for cautious removal of the foreign body, with the assistance of a rare earth magnet or forceps extraction, depending on whether the material is ferromagnetic or not.

Lesions
Retinal laser is applied in a focal or scatter array. It serves to seal leaking blood vessels, destroy ischemic retina or burn the RPE to create a scar. The greatest risk is a laser is a miss-hit to the center of the macula (fixation). Tolerances are close: many diabetic lesions are only 200 microns (1/5th of a millimeter) from fixation. If the burn is too intense, there may be lateral heat spread and collateral retinal damage and too much damage to the RPE setting the stage for choroidal neovascularization and scarring. Collateral retinal damage may become manifest months to years after the laser application. Heavy scatter peripheral laser may impair peripheral vision and visual field and lead to complaints of night blindness.
