Green Hills Health & Rehabilitation Center

This place is a place that every one who has to go to a nursing home fears. It was terrible. Bed bugs, unclean procedures in dealing with patients with C-dif , no soap or paper towels in the rooms for nurses to even wash their hands. The managment is clueless and doesn't care. I even showed the manager video of what was going on and got very little response. He couldn't even remember if I had my mother or my father in the facility. If you love someone never send them there don't let the name of green hills fool you. I still have nightmares about this place and feel that I totally let my parent down by making their last days on earth so bad.

This place needs to be shut down. Nasty acting nurses, and nurse management! Unprofessional workers. Will NEGLECT your loved one. It's all about MONEY!

Horrible - Do not send your loved one here. Place is dirty and the food is horrible. They are very short staffed. When patient calls for the nurse they hardly ever come.

Some on floor staff very caring nursing, aids.physical therapy. Bad news is dietary sucks,care is unclean,picking meds off dirty floors to give to patients,not getting patient up daily, leaving elderly expose to the cold,poor use of gloves, empty Foley in dirty garbage can,mold on bath room ,dirty sinks, floors not washed in rooms daily. No food choices no special diet or choices despite what they say. Social worker,person who schedule app,asst.nsg manager all appear nice but are not very truthful,controlling manipulative and really don't advocate for the patients or try to help pt with getting other services. All I can say think twice about putting love ones here. If no other choice visit regularly unannounced times never give them a schedule. If you feel something is wrong it most likely is get them out of there. I left AMA after they missed serveral of my appt with my doctor and would not talk to me about it. Rate them less than one.
