Cryotherapy
The most effective proven treatments for ROP are laser therapy or cryotherapy. Laser therapy "burns away" the periphery of the retina, which has no normal blood vessels. With cryotherapy, physicians use an instrument that generates freezing temperatures to briefly touch spots on the surface of the eye that overlie the periphery of the retina. Both laser treatment and cryotherapy destroy the peripheral areas of the retina, slowing or reversing the abnormal growth of blood vessels. Unfortunately, the treatments also destroy some side vision. This is done to save the most important part of our sight—the sharp, central vision we need for "straight ahead" activities such as reading, sewing, and driving.

It is theorized that vein occlusions occur when there is compression of a retinal vein by a neighboring retinal artery. Over time it is believed that artery walls can become thicker and stiffer. In areas where veins and arteries travel together, they are surrounded by a sheath of tissue, so the vein becomes compressed by the thickened artery. This in turn leads to turbulence in the vein that eventually results in the formation of a clot which blocks blood flow. Risk factors for the development of vein occlusions include hypertension, smoking, diabetes and glaucoma.
Ophthalmology
Dr. Fern is a native New Yorker. He earned his Bachelor of Arts degree from Emory University in Atlanta, Georgia, where he was elected to Phi Beta Kappa. He then received his medical degree from New York University School of Medicine. He completed a residency program in Ophthalmology at NYU Bellevue Medical Center, followed by a fellowship in Vitreo Retinal diseases and surgery at the University of California, Davis Medical Center. Dr. Fern is the Chief of Ophthalmology at Northern Westchester Hospital and former directer of retinal surgery at Bellevue Hospital, the main teaching hospital of New York University School of Medicine. Dr. Fern has been practicing in Westchester since 1996. He is a Board Certified Ophthalmologist, as well as an active member of the New York State Ophthalmology Society, the American Academy of Ophthalmology and the American Society of Retinal Specialists. Dr. Fern is listed in Castle Connolly's best ophthalmologists for Westchester County.
Macular Degeneration
Anti-VEGF Therapy VEGF (Vascular Endothelial Growth Factor) is a secreted protein that assists in the development of new blood vessels. Blocking the VEGF protein is beneficial in a number of diseases. In Wet AMD (Age Related Macular Degeneration), anti-VEGF drugs such as Avastin, or Lucentis help to block the growth of new blood vessels, and can prevent the progression of the disease. In many cases, vision can actually be improved with timely and effective treatment. VEGF drugs can also be beneficial in the treatment of some cancers. The theory is that if you cut off the development of new blood vessels, you "starve" the tumor and prevent the spread of the disease. Avastin is an example of one of the cancer drugs.
Cataracts
Ophthalmic ultrasonography uses high-frequency sound waves, which are transmitted from a probe into the eye. As the sound waves strike intraocular structures, they are reflected back to the probe and converted into an electric signal. The signal is subsequently reconstructed as an image on a monitor, which can be used to make a dynamic evaluation of the eye or can be photographed to document pathology. This instrument is extremely valuable when Dr. Fern’s view inside the eye is obstructed by blood, an extremely dense cataract, or other cloudy media.
Cataract Surgery
There are many causes of macular edema. The most common causes are diabetes, inflammation, macular pucker and vein occlusions. One particular kind of edema called cystoid macular edema can occur after cataract surgery or from other causes of inflammation.
Eye Exam
An eye examination where drops are placed in your eyes to widen, or dilate, the pupils. Your eye care professional uses a special magnifying lens to examine your retina and optic nerve for signs of damage and other eye problems. After the exam, your closeup vision may remain blurred for several hours.

A macular pucker can also be triggered by certain eye diseases and disorders, such as a detached retina and inflammation of the eye (uveitis). Also, people with diabetes sometimes develop an eye disease called diabetic retinopathy, which can cause a macular pucker. A macular pucker can also be caused by trauma from either surgery or an eye injury.

Fungus
Ocular Histoplasmosis is a major cause of visual impairment in the eastern and central United States where 90 percent of adults have been exposed to histoplasma capsulatum. This common fungus is found in molds from soil enriched with bat, chicken or starling droppings and yeasts from animals.
