The Sleep Center consists of board certified staff that are experts in the diagnosis and treatment of sleep disorders. The Sleep Center was one of the first accredited sleep disorder centers in the State of Indiana. Both in-house sleep studies (polysomnograms) and home sleep studies are performed on the appropriate patients. The Center houses a dedicated CPAP clinic for those patients who are treated with positive airway pressure. The national average for CPAP compliance is only 43%. The Sleep Center’s compliance at the sleep clinic is near 90%.

There are approximately 100 different sleep disorders that, if not diagnosed accurately and treated properly, can cause the following symptoms: excessive daytime sleepiness, insomnia, mood disturbances, hypertension, heart disease such as heart attacks and/or congestive heart failure, stroke, atrial fibrillation and other cardiac arrhythmias, attention and concentration problems (looking much like ADD/ADHD in children), as well as glaucoma, memory problems, and sexual dysfunction.

Dr. Guevara graduated from the University of the Philippines, Manila (MD 1990). He completed his Internal Medicine Internship at the Good Samaritan Hospital (Baltimore, MD 1992) and a Neurology Residency at the State University of New York (SUNY) in Syracuse, NY (1992-1995). Dr. Guevara then completed two (2) Fellowships in Neuro-ophthalmology, the first (1995-1996) at the University of Illinois at Chicago and the second (1996-1997) at Emory University in Atlanta, Georgia.

He is board certified in neurology by the American Board of Psychiatry and Neurology (ABPN) with added qualification in clinical neurophysiology. He has also earned board certifications from the American Board of Electroencephalography, the American Society of Neurorehabilitation, and the American Board of Sleep Medicine.

Ms. Starcevich is an adult nurse practitioner who is highly experienced in the care of patients with head injuries, stroke, and other central nervous system debilitations. Some medical conditions can cause drowsiness. The most common is diabetes. Other conditions that may lead to drowsiness include those that cause chronic pain or affect your metabolism, such as hyponatremia (when the level of sodium in your blood is too low), or hypothyroidism. Finally, infectious mononucleosis (Mono) and chronic fatigue syndrome (CFS) are other well-known causes of drowsiness.

During a physical exam a doctor can determine whether the tremor occurs primarily during action or at rest. The doctor will also check for tremor symmetry, any sensory loss, weakness or muscle atrophy, or decreased reflexes. A detailed family history may indicate if the tremor is inherited. Blood or urine tests can detect thyroid malfunction, other metabolic causes, and abnormal levels of certain chemicals that can cause tremor. These tests may also help to identify contributing causes, such as drug interaction, chronic alcoholism, or another condition or disease. Diagnostic imaging using computerized tomography or magnetic resonance imaging may help determine if the tremor is the result of a structural defect or degeneration of the brain.

Injuries. Rear-end auto collisions often result in whiplash injury, which occurs when the head is jerked backward and then forward, straining the soft tissues of the neck.
Pinched Nerve
Electromyography (EMG). If your doctor suspects your neck pain might be related to a pinched nerve, he or she might suggest an EMG. It involves inserting fine needles through your skin into a muscle and performing tests to measure the speed of nerve conduction to determine whether specific nerves are functioning properly.
Neurosurgery
These are wild flinging/throwing movements of one arm or leg, usually occurring as a result of a cerebrovascular event. They can vary in intensity from mild to severe and may even cause injury. They usually subside over a period of 3-6 months but can be treated with a phenothiazine, haloperidol or tetrabenazine. They may require neurosurgery to be adequately controlled.

Postural – occurs if a limb is static (can also remain during movement). Types include physiological tremor, exaggerated physiological tremor – eg, in thyrotoxicosis, anxiety states, alcohol abuse, drugs (eg, sympathomimetics, antidepressants, valproate, lithium), heavy metal poisoning (‘hatter’s shakes’ from mercury). Neurological disease – eg, severe cerebellar lesions, Wilson’s disease, neurosyphilis, peripheral neuropathies, benign essential (familial) tremor, task-specific tremors (eg, primary writing tremor).

Physical Therapy
Physical therapy may help to reduce tremor and improve coordination and muscle control for some individuals. A physical therapist will evaluate the individual for tremor positioning, muscle control, muscle strength, and functional skills. Teaching the person to brace the affected limb during the tremor or to hold an affected arm close to the body is sometimes useful in gaining motion control. Coordination and balancing exercises may help some people. Some therapists recommend the use of weights, splints, other adaptive equipment, and special plates and utensils for eating.
