Medical Weight Loss
Type 2 diabetics have a metabolically complex disease, and various medications are available to address these problems. Sulfonylureas are the classic diabetes medications such as glipizide, glyburide and glimeperide. They work by promoting the secretion of insulin by the pancreatic islet cells. They are very effective at lowering blood glucose, but since they increase insulin production, they have the risk of causing hypoglycemia (low blood glucose). Biguanides include metformin, perhaps the number 1 selling diabetes medication in the world. It works by minimizing the production of extra glucose from the liver, and improves somewhat the body’s response to insulin. It commonly causes a decrease in appetite, in part due to some nausea. Because of this, patients often note a weight loss with this medication. TZD drugs such as pioglitazone and rosiglitazone are very effective at improving insulin sensitivity to lower blood glucose. Incretin drugs help your body make more insulin and decrease glucose production from the liver. This include DPP-4 inhibitors (sitagliptin, saxagliptin, linagliptin) which are taken orally and GLP-1 receptor agonists which are injectable (exenatide, exenatide extended release, and liraglutide). GLP-1 RA drugs slightly delays the transfer of food from the stomach to the intestines, so the stomach feels full sooner. Though sometimes causing nausea,GLP-1 RA drugs also suppress appetite in some patients, thereby causing potential weight loss. SGLT-2 inhibitors (canagliflozin, dapagliflozin) increase release of glucose through the urine when blood sugars are high and thereby lower blood glucose. Several other anti-diabtetic drugs are currently approved in the US and can be utilized by your endocrinologist when necessary to optimally control your diabetes.

She worked as a Nuclear Medicine physician and Associate Professor in Tehran, Iran for 14 years prior to immigrating to The United States of America. She completed her U.S. certification as a Physician Assistant at Touro University Nevada. She practiced Family Medicine for 2 years and ultimately joined Desert Endocrinology in 2013. Flu Shots
Both types of diabetes result in a higher risk of cardiovascular disease throughout the body. This may cause a greater risk of heart disease, stroke, kidney failure, blindness, impotence, amputation and other diseases. Because of the higher risk of complications, our endocrinologists strive to prevent them, rather than wait until they occur. Treatment therefore includes appropriate lowering of blood sugars but also use of other medications such as statins and aspirin to lower cardiovascular disease, and ACE inhibitors to lower the risk of kidney failure. Education, exercise, diet and weight management are also important parts of maintaining the health of a diabetic patient. Other measures may include frequent foot exams, yearly eye exams for retinal damage and yearly flu shots. Daily home blood sugar monitoring is important to determine the response to therapy and allow for more precise medication adjustments. Studies have shown many of the dreaded complications of diabetes can be minimized or avoided by proper care and patient compliance.
Pneumonia
Fracturing may occur in any bone, but the areas of most concern have been the spine, hip and forearm. While fractures rarely are a cause of death directly, they cause other major problems. Rounding of the back, or “dowager’s hump,” may occur after multiple crush fractures of spinal vertebrae bones. These patients cannot stand upright and may not fully expand their lungs. They will have difficulty reaching upwards as well. Hip fractures may result in being bedridden for weeks or months. This in turn may increase the risk of blood clots or aspiration pneumonia, which could be deadly. Many of these patients never regain their independence, and reside in a nursing home. Forearm fractures are often quite painful.

Eye Exam
Because diabetes is said to be the major cause of adult blindness, a yearly eye examination with photos of the retina are important for all diabetics. The retina is the light sensitive layer in the eye, which forms the images we see. Bleeding and scarring can damage the retina, causing a loss of vision.

Pregnancy
Jamie Jadid is board certified as a Family Nurse Practitioner and earned a Master of Science in Nursing from the University of Nevada, Las Vegas. She also holds certification in Advanced Diabetes Management and is a Certified Diabetes Educator. She joined Desert Endocrinology in July of 2018. Previously, she was employed at the High-Risk Pregnancy Center where she spearheaded their diabetes consulting program, providing care for women with diabetes in anticipation of, during, and after pregnancy. She served as the coordinator of their Diabetes Education Program that was recognized by the American Diabetes Association for meeting their program requirements. Jamie has been a guest speaker at national conferences speaking on the pathophysiology and treatment of diabetes, diabetes management in diverse populations and diabetes in pregnancy. Jamie’s practice is not limited to diabetes care but also includes use of diabetes technology and general endocrine care.

Hypothyroidism
We see many patients that come to us with persistent symptoms of hypothyroidism despite “normal thyroid function tests” according to their healthcare provider. Similarly, many patients have symptoms of hypothyroidism when their thyroid tests are interpreted as “normal” by their healthcare providers. This is one of the most frustrating situations any patient will ever encounter. In some cases, after a careful review of the patient and in-depth thyroid testing, we are forced to conclude “it’s not your thyroid.” However, in many cases it is the thyroid. Still, in other situations it is another unrecognized endocrine disorder.

Thyroid Cancer
Thyroid Cancer is a malignant tumor involving the thyroid gland, and the most common endocrine cancer. In contrast to most other cancers, the incidence of thyroid cancer is increasing, with approximately 45,000 new cases being diagnosed in the US every year. Nevertheless, thyroid cancer is a highly treatable cancer that enjoys excellent prognosis with appropriate care

Ultrasound
Other tests such as heel ultrasound, heel DEXA or finger DEXA are helpful in screening, but are not generally used for follow up assessment as your doctor retests your bones to see if therapies have improved your bones.
MRI
The vast majority of adrenal nodules (also referred to as adrenal adenomas) are benign. It is still very important to exclude cancer in each patient with an adrenal adenoma. Malignant potential is best evaluated by radiographic diagnostic imaging using CT scanning or MRI. Size of the adrenal nodule is a very important consideration. Over 90% of adrenal carcinomas are greater than 4 cm in size. But, since adrenal cancer is uncommon, most large adrenal nodules are still benign. It is usually necessary to repeat the diagnostic imaging of the adrenal nodule 3–6 months after its initial discovery. Nodules that increase in size over a 3–6 month period of observation are also worrisome. Most experts recommend surgical resection of a nodule that is > 4 cm in size or if the nodule shows a significant increase in size during surveillance.

Computed Tomography
Testing for bone density is most commonly done using a DEXA scan, which uses very low dose x-rays (about 1/10 of a chest x-ray) to measure bone calcium content. Scans of the hip, lumbar, spine and sometimes the forearm are commonly done. Follow up scans done in 1–2 years may help determine if efforts to build bone density have helped. QCT or quantitative CT scan is also done, but traditionally it involves much more radiation than the DEXA, and cannot measure hip density. It is much less commonly used than DEXA.
Chemotherapy
Low Testosterone (“Low T”) – presents as fatigue, loss of interest in sex, feeling depressed, along with thinning of bone density, loss of muscle mass and loss of body hair. While men do not have a common age of “male menopause”, as females do, there is a decline in male hormones as a part of the aging process. Other causes may include testicular failure due to disease or trauma, pituitary disease as well as side effects from chemotherapy or radiation. Because testosterone therapy may have important side effects, the diagnosis of a low testosterone, and any decision for therapy must be carefully judged by physician and patient.

Ingrown Toenails
While the short-term complications of poorly controlled blood glucose levels can involve increased urination and dehydration, the long-term complications are principally circulation damage and nerve damage. For example, the combination of poor circulation and numb feet is particularly risky. Such as patient may suffer a simple cut or foot blister, which becomes infected. If the infection goes deeper, into the bone, the infection may not respond to antibiotics and an amputation may become necessary. Because the patient’s feet are numb, they are oblivious to even the early signs of infection. Therefore we recommend daily foot inspections by patients. We recommend they call our office or their foot care provider if they find any signs of infection or injury. Even a simple ingrown toenail can worsen, resulting in an amputation.
