        The Nelson Academy staff has been in the healthcare industry a minimum of 15 years. Upon completion of our program, our students go on to work in group homes, nursing homes, skilled facilities, home health care and hospitals. We have a broad spectrum of knowledge that is pertinent for providing assistance to the community. We are ecstatic at the opportunity to share our Nursing Assistance Training experiences with others. Completion of 9th Grade Reading (English)/Math Levels 2. Childhood Immunization/Shot Record (If Unable To Locate, Bloodwork May Be Drawn (Titers) To Assess Immunity) 3. Physical Form Complete By MD 4. No Felony Convictions (See Board Of Nursing Specifics Www.Azbn.Gov) 5. Parent/Legal Guardian Consent If Under 18 Years Of Age 6. Social Security Card 7. Photo Proof of Identity (State Id/Driver License/Passport) 8. Pay Non-Refundable Enrollment Fee 9.
