Male Infertility
Intracytoplasmic sperm injection (ICSI) has revolutionized the treatment of male factor infertility. ICSI can be used to treat a variety of male infertility disorders, such as low sperm count (oligospermia), low sperm motility (asthenospermia), and abnormally-shaped sperm (teratospermia). Additional conditions that may require ICSI include: problems with sperm binding to and penetrating the egg, prior or repeated failure with conventional IVF methods, and obstruction of the reproductive tract.
Infectious Diseases
Fort Worth Fertility acquires cryopreserved (frozen) sperm only from reputable, licensed sperm banks. Semen is frozen in liquid nitrogen at a temperature of -196°C, where all sperm activity is essentially halted until it is thawed. After thawing, most sperm return to the pre-freeze motile and functional state. To insure safety, the sperm donor is tested for HIV and other infectious diseases at the time of sperm donation, and the sperm is then frozen for six months. The donor is retested at this time, and if the donor's testing for infectious diseases remains negative, the frozen sperm is made available for use. While this does not eliminate the possibility of disease transmission, it makes the risk extremely low.
Electrocardiogram
Required laboratory and diagnostic studies include a recent pap smear, TSH, mammogram if necessary, hysteroscopy and a trial transfer. If the patient is older than 44 we also require an EKG, stress test, chest x-ray and evaluation by a primary care physician.

Female Infertility

Thyroid

Ultrasound
In general, an ultrasound is performed prior to initiation of clomiphene citrate in order to ensure that this medication can be given safely. Women occasionally have an ovarian cyst, which may prevent the use of clomiphene citrate during that cycle secondary to possible negative effects of the cyst on ovulation. Intercourse should begin 3-5 days after the last clomiphene citrate tablet every other day for approximately 1 week.

Previous chemotherapy, ovarian surgery, premature menopause and absence of ovaries at birth are some of the reasons why ovaries are absent or function poorly. Additionally, some women carry a genetic disorder they do not want to transmit to a child.
Radiation Therapy
These circumstances stem from a number of causes such as the absence of sperm production, previous vasectomy, or residual effects from chemotherapy or radiation therapy. Donor Sperm insemination may also be used in cases where both the male and female are carriers of a genetic disorder or a female is severely Rh immunized and the male is Rh positive.

Laparoscopy
The important thing is for the surgeon to identify that there is at least 4-5 cm of Fallopian tube to work with (average length of fallopian tube is 6-10 cm). If there is any doubt regarding whether a tubal reversal could be considered, we will first perform a laparoscopy to evaluate the quality and length of tube remaining and if there is a sufficient length of tube, he will proceed with the tubal reversal.
