The webinar was entitled, "Otoscopic Diagnosis for the Primary Care/Emergency Room Physician." The webinar reviewed the various ways to diagnose outer and middle ear diseases through an examination of the ear using a handheld otoscope or microscope.
Ear Infections
How Does It Occur? A cholesteatoma usually occurs because of poor eustachian tube function as well as infection in the middle ear. The eustachian tube conveys air from the back of the nose into the middle ear to equalize ear pressure. When the eustachian tubes work poorly, perhaps due to allergy, a cold or sinusitis, the air in the middle ear is absorbed by the body, and a partial vacuum results in the ear. The vacuum pressure sucks in a pouch or sac by stretching the eardrum, especially areas weakened by previous infections. This sac often becomes a cholesteatoma. A rare congenital form of cholesteatoma (one present at birth) can occur in the middle ear and elsewhere, such as in the nearby skull bones. However, the type of cholesteatoma associated with ear infections is most common.

Diagnosis As with a glomus tympanicum, the diagnosis of a glomus jugulare tumor is made based on symptoms, physical findings, hearing tests and a CT of the ear/temporal bone. Physical examination may also show a reddish/bluish mass behind the ear drum (See figure). In the case of a glomus jugulare, the mass behind the ear drum is really just the “tip of the iceberg”. An MRI is also ordered to examine the size of the tumor in relationship to the surrounding soft tissue of the brain or blood vessels. Blood and urine tests are done to determine if the tumor is secreting adrenaline-like hormones. These “active” tumors are treated with special high blood pressure medications prior to surgery to prevent anesthetic complications.
Radiology
Complete microsurgical excision of a glomus jugulare tumor requires more extensive surgery than that for glomus tympanicum tumors. This is an exciting time because advances in surgical technique, imaging technology, and interventional radiology over that past four decades have enabled the removal of previously unresectable tumors. Surgery is often performed through incisions extending from the scalp down into the neck. Sometimes the middle ear hearing bones are removed, which causes a partial hearing loss. Depending on the size of tumor, structures such as the jugular vein, carotid artery, cranial nerves, eustachian tube, and the temporomandibular joint (jaw joint) may be exposed. Large tumors may also require a craniotomy (exposing the brain) by a neurosurgeon to remove tumor touching the brain. Hospital stay is often several days when complete microsurgical removal is performed.

What Treatment Can be Provided? Initial treatment may consist of a careful cleaning of the ear, antibiotics, and ear drops. Therapy aims to stop drainage in the ear by controlling the infection. After the infection is controlled, surgical removal is often recommended to prevent serious complications. A hearing test is performed, to help estimate if there is any damage to the middle ear bones or inner ear. A CT scan is often recommended to determine the size of the cholesteatoma. Surgery is performed under general anesthesia in most cases. The primary purpose of the surgery is to remove the cholesteatoma and achieve an infection-free, dry ear. Hearing preservation or restoration is the second goal of surgery. In most cases, patients are discharged on the same day of surgery.
Radiation Therapy
With standard external beam radiation therapy, tumors and much or all of the surrounding brain are treated with the same dose of radiation. Standard external beam radiation is given in small increments over several weeks to allow normal brain tissue to recover from the harmful effects of the external beam radiation. The brain can only absorb a certain dose of radiation, beyond which no further treatment is advisable.

A variety of surgical services are provided by the Ear Institute of Chicago. The types of procedures performed are designed to treat hearing, balance and facial nerve disorders. The complexity of the procedures varies from placement of tympanostomy tubes (ear tubes) to more complex multidisciplinary skull base surgery, for example, removal of acoustic tumors or glomus jugulare tumors. Ear Surgery
Operations on the ear may also cause an injury to the facial nerve. Facial paralysis after ear surgery, fortunately, is very uncommon. Facial paralysis may occur, however, when the facial nerve is not in its normal anatomical position (congenital abnormality), or when the nerve is so distorted by the mastoid or middle ear disease that it is not identifiable. In rare cases, it may be necessary to remove a portion of the nerve in order to eradicate the surrounding disease.
