Fluoride Treatment

Fillings
This patient had a very large amalgam filling removed after it cracked. There was not enough tooth remaining to place another filling. Either a crown or porcelain onlay would have been appropriate treatment. We chose the onlay due to its conservative nature. The final restoration can be seen on the right, just after it was completed. This restoration was done using the CEREC CAD/CAM method.
Resin Composite
This patient had pit defects in the enamel layers of his upper six teeth. Since he was a smoker these pits collected the smoke stain and looked awful. Microabrasion successfully removed the stains and smoothed out the pits. The deepest pits were filled with composite resin bonding material to slow down the accumulation of stain. The bonding over time will need to be replaced.

Crowns
With a microscope, dental procedures can be performed more accurately and reliably due to the increased magnification and shadow-free light. Our operating microscope can increase magnification up to 25x, expanding our ability to diagnose and treat the most minute dental problems, enabling us to truly provide the most minimally invasive dental care possible today. For example, fissure caries and microcracks cannot be seen with the naked eye but are highly visible with the aid of a microscope. What once would have needed a crown, can frequently be restored with smaller restoration when microscope assisted preparation is combined with a CAD/CAM restoration.
Bridges
As my Office Manager says, "Oy-vey...what a mess!" If you can believe it, this man was seen regularly by a periodontist and orthodontist for over twenty years and was never guided to a dentist. He saw me only after he was unhappy with another dentist's temporary bridge on the upper right side. After four months of treatment, we rebuilt his back teeth with porcelain fused to metal crowns and bridges and bonded his front teeth to replace the missing tooth structure. Notice on the lower front, we placed pink colored porcelain to create an illusion of normal sized teeth where the bone and overlying gums were gone due to traumatic extraction years earlier. We could have alternatively regrown both the bone and gums using surgical techniques, but this patient was not interested in this type of surgery.
Dental Bonding
Tetracycline Staining is one of the most difficult discolorizations to treat. Long term bleaching (4-6 months), can be effective for light and moderate staining. For severe staining either cosmetic bonding or porcelain laminates are the best treatment. Newer "luminary porcelains" can be used to create vital looking teeth without the more common opaque look.
Restorative Dentistry
This website is named Dental Art and Science because Dentistry is no longer just about filling teeth. New technology and materials have evolved dentistry as a science with functionally esthetic outcomes. All dentistry must function within the limits of the oral system or it will either cause harm, or fail prematurely. All materials used have good and bad properties and their selection and use must be carefully understood if a long lasting, functional and beautiful result is to be achieved. Root Canal Treatment
This patient is a model and had porcelain laminates done by another dentist. It is evident why she was not happy with her smile. Beside the color and staining, notice the difference in height of the two central incisors. Initially I recommended gingival surgery in order to even the height of the two central incisors, but she did not want to have surgery. So instead, using knowledge of tooth anatomy and contour we were able to create an esthetic form and color. We placed laminates on her six upper front teeth as well as her on lower right central incisor, which was discolored after a root canal treatment.

Periodontal Surgery
This patient was not very happy with her previous dentist either. The bridge on her upper right was only six months old and the tooth which looks backward (left lateral incisor) was going to be extracted. Look how unhealthy the gums appeared above the recent bridge and notice how uneven the gumline was from left to right...I won't mention to bad bonding. After extracting the failing lateral incisor, doing periodontal surgery to correct boney and gingival problems and placing two three tooth bridges, she was happy and healthy!

Dentures
This patient also had a combination approach taken. She had two crowns, three porcelain laminates and a removable partial denture fabricated in order to enhance her smile.

Dental Implants
By using Sirona's ® CAD/CAM technology and 3D X-ray technology (Cone-Beam CT) we can now fully integrate these data sets into one single design screen, allowing us to reverse engineer implant cases and place implants in positions which avoid vital anatomical structures (nerves and sinuses) yet still allow for ideal life-like tooth restorations. Implant surgical guides are milled in-office or 3D printed. These guides provide precision of 0.1mm from the virtual to the actual surgery.

Oral and Maxillofacial Surgery

Oral Surgery
Gum Surgery
This patient had a myriad of issues with her smile. Her main complaint was that she didn't like her smile. Her teeth were too dark, they were rotated and the old bondings were discolored. We staged her treatment. The first step was to do gum surgery in order to even the heights of her teeth. Then we bleached her teeth in order to lighten the overall shade. Next, we placed five porcelain laminates, the four center teeth (central and lateral incisors), as well as her left first premolar. The premolar is tucked into a shadow in her preoperative photograph. This type of combination approach frequently gives the best outcome. Each tooth needs to be individually assessed for its needs, then the entire smile needs to be evaluated in relationship to the lips and face.
Tooth Extractions
Bone Grafting
This patient had a previous bridge which got decay underneath the margin (see first picture). The teeth supporting the bridge needed to be extracted because they could no longer support a bridge. This patient did not want a removable denture. Three implants were placed in the bone after bone grafting procedures were completed by a local Oral Surgeon (second picture). After Six months, the implant were uncovered and a new bridge was inserted. This bridge was screwed into the implants so that it could be easily removed if necessary. The screw access holes can be seen filled with tooth colored composite resin in the final photograph.

Orthodontics
This patient is the daughter of the previous patient. Notice the similarities of their teeth. When her mother came home, she saw what could be done, then asked me to bond her teeth too. You can notice the severe wear of her front teeth. We cosmetically contoured some teeth, then bonded the central incisors it close the space (diameter). She would benefit from orthodontics, but did not have the resources.
Braces
As you will see in the following cases, I have treated cases as extreme as severe fluorosis and as simple as a small white line after braces have been removed.

Teeth Whitening
Teeth which are in the yellow or yellow-orange range are 75% successfully treated with home bleaching or home and in-office bleaching. The procedure is painless and relatively inexpensive. It does not damage the tooth structure and only requires occasional re-bleaching. This patient bleached her teeth using a 16% carbamide peroxide gel in a custom formed bleaching tray in her own home. Her result was obtained in four weeks of bleaching one hour per day.
Cosmetic Dentistry
There are many types of crowns on the market today, each with its pros and cons. Ask your dentist which is most appropriate for your situation. In this section you will see examples of porcelain fused to metal crowns, porcelain fused to metal crowns with porcelain margins and all ceramic crowns. Each has its place in cosmetic dentistry.
