Johnson Adult Day Program (JADP) provides a safe, stimulating environment for adults with memory loss or physical impairment. Who is it for? Adults with memory loss and dementiadue to Alzheimer's, Parkinson's or other conditions; Adults with physical impairment from health challenges like stroke or complications due to COPD; Adults needing palliative care or monitoring for a medical condition. Seniors Inc. provides a home services, information and support to help aging individuals live safely and comfortably at home. Who is it for? People who need help with activities of daily living to maintain a safe, healthy living situation; People recently discharged from a hospital living situation; People who live in the Denver metro area, Canon City and Trinidad. Total Longterm Care (TLC) provides coordinated healthcare, home care and support to help aging individuals live independently in their community. Who is it for? People who are 55 years old or older and live in one of the TLC's service areas; People who are certified by the state of Colorado as meeting the need for nursing home level of care; People who are able to live safely in the community with the help of TLC.
