Remember that ophthalmologists are physicians who have completed medical school and who have training not only in eye problems but also in other bodily illnesses in which the eyes may be involved. Within our practice, Drs. Turtel and Pardon are fellowship-trained subspecialists in pediatric ophthalmology.
Pediatric Care
There are many children who have difficulty reading, reverse their letters or are generally slow at learning. We often receive referrals from pediatricians for complete evaluation of these children's eyes. Certainly this is recommended, for if a child cannot see well, learning may be impaired. However, true learning disabilities such as dyslexia are virtually never based on eye disease. Furthermore, they are NOT improved by the so-called eye muscle exercises that have become popular in recent years. Eye muscle problems do not create learning difficulties. Children do not reverse letters (such as b and d) because of eye problems. Both the American Academy of Pediatrics and the American Academy of Ophthalmology want you to know that these eye exercises are of no value. If your child has learning difficulties, the problem should be left to the learning disabilities specialists and the educators, NOT eye care providers who prescribe eye exercises. Please don't hesitate to talk to our specialists about this important subject.
Primary Care
The retina is the only part of the eye in which we can actually look directly at blood vessels. For this reason, examination of the retina may give some information about one's general health. For example, if you are a diabetic, changes from diabetes might be observable by examining the retina. The same is true for high blood pressure. Keep in mind, though, that if the retina appears normal, this does not mean that you do not have diabetes or high blood pressure, only that it hasn't shown up in your eyes. You still need regular medical examinations by your primary care physician.

Ophthalmologists

Glaucoma
When ophthalmologists look at the retina, it is often done with the pupils dilated. This simply allows better visibility by making a larger opening through which we examine it. There may be retinal problems that only are present at the very outer edge of the retina, and these often cannot be seen without dilating drops. In addition, a larger opening permits a better three-dimensional view. This is important in evaluating for other diseases such as holes in the retina. Retinal holes and peripheral retinal thinning may lead to retinal detachment, and it is far better to detect problems early when they are more readily treated. Glaucoma, which affects the optic nerve is often detected by the very same type of examination. And retinal tumors, while not very common, may only be visible through a dilated pupil as well. Optomap ultra-wide field retinal imaging is now available and can be used in many patients to avoid dilation and provide the doctor with additional information about your eye health.
Ophthalmology
Dr. Talansky, a native of Clifton, N.J.,received his undergraduate degree from Rutger's University. After being awarded his Doctor of Medicine degree from the Medical University of South Carolina, and being elected to Alpha Omega Alpha, the National Medical Honor Society, he remained to intern and complete his residency training in ophthalmology at the Storm Eye Institute there. Later, he returned to the Storm Institute to complete a postgraduate fellowship program in Retinal Diseases and Surgery. Dr. Talansky is also certified by the American Board of Ophthalmology and is a Fellow of the American Academy of Ophthalmology.
Macular Degeneration
The retinal disease that most older patients worry about is macular degeneration. At the present time, there is no known specific cause for this problem. There are different types of macular degeneration, and they are managed differently.
Laser Eye Surgery
Modern laser vision correction is performed with the use of a highly advanced laser known as an excimer laser. Procedures such as LASIK, LASEK, epi-LASIK, PRK, and Advanced Surface Treatment or Ablation all utilize this type of laser. It is now available for many patients who could not previously be corrected by RK. This includes many nearsighted patients, especially the younger ones who have higher levels of nearsightedness. These techniques are performed on an outpatient basis, and the results have been very gratifying. More recently with improved enhancements in laser technology, nearsightedness, both lower and higher levels, with or without astigmatism, and some levels of farsightedness can now be corrected more safely and effectively than ever before.
Cataracts
A cataract is a clouding of the human eye's natural lens. Affecting more than 22 million Americans, it is the most common cause of vision loss in people over age 40.
Cataract Surgery
When symptoms begin to appear, you may be able to improve your vision temporarily by using stronger glasses, magnification, appropriate lighting or other visual aids. Cataract surgery is your only treatment option when your cataracts have progressed enough to impair your vision and affect your activities of daily living. Many people consider poor vision an inevitable part of aging, but cataract surgery is a simple, relatively painless procedure to regain your vision at any age.
Eye Exam
Our Contact Lens department, under the direction of our optometrist, Dr. Heidi Schulman, fits all types of contact lenses, clear and colored, including conventional soft lenses, disposables, and toric lenses for astigmatism. Multifocal contact lenses are also available so that in many cases, the need for reading glasses may also be reduced or eliminated. When indicated, we also fit the latest gas-permeable hard contact lenses both for cosmetic wear and for treatment of certain eye diseases. Prior to you being fit, we will perform a thorough eye examination to ensure that there are no eye diseases that might be a problem with contacts and that we can expect the kind of clear comfortable fit that you expect. Our fitting techniques are individualized to your own personal needs, and you will be assured that we will follow up on the initial fitting to make certain that there are no problems that could lead to later difficulties.

Internal Medicine
Dr. Patel is a lifelong NJ resident who earned her Doctor of Medicine degree at Robert Wood Johnson Medical School where she was elected to Alpha Omega Alpha and the Gold Humanism Honor Society. Dr. Patel completed her internship in Internal Medicine at Robert Wood Johnson University Hospital prior to completing Ophthalmology residency at Medstar Georgetown University Hospital in Washington, DC.

Diabetes Care
Diabetic Retinopathy
There are many disorders of the retina itself which can interfere with its ability to see light. Hemorrhage such as in diabetic retinopathy, swelling from injury, age-related hardening of the arteries and its consequent poor circulation and toxic chemicals or drugs are just a few of the problems that may decrease a person's vision. Just as putting a new lens onto a camera which has an old, outdated roll of film will not improve the picture, changing one's eyeglasses will not improve the vision if it is the retina which is diseased or injured.

Reconstructive Surgery
The specialty of oculoplastic surgery includes cosmetic and reconstructive surgery of the eyelids and peripheral eyeball tissues. Most commonly known is the operation called blepharoplasty. This is the procedure for removing excessive eyelid skin and reducing puffy lids. Sometimes it is called an eyelid "tuck." While many blepharoplasties are done purely for cosmetic reasons, there are some visual conditions for which this may be helpful. For example, as we age, there is often a redundancy of the upper eyelid skin which results in a large fold of sagging skin that hangs down and obstructs the upper portion of the vision. Removal of this skin (and occasionally the fat pad behind it) by a blepharoplasty is done for a medical eye problem and may be covered by insurance.

Plastic Surgeons

Botox
BOTOX is a medication that works by blocking nerve impulses to injected muscles. In 1989, Botox was first approved by the U.S. Food and Drug Administration for the treatment of patients with crossed eyes, eyelid spasms and partial facial muscle spasms. Ophthalmologists were the first to use Botox, and still use it frequently, for these conditions. In 2002, the FDA approved the use of Botox for treatment of severe wrinkles between the eyebrows (glabellar area). It has been used with great success treating forehead and "crows feet" wrinkles as well. Our practice has extensive experience with the usage of Botox for both medical and cosmetic purposes.
Plastic Surgery
Other types of reconstructive eyelid plastic surgery would include ptosis repair (surgery to elevate a drooping eyelid), tear duct reconstructive procedures for tear duct obstruction that is associated with constant watering of the eye(s), and removal of eyelid tumors, both benign and malignant. Positional problems such as entropion, a condition in which the edge of the eyelid rolls inward causing the lashes to rub against the eyeball, or the opposite, ectropion, in which the edge of the eyelid falls away from its contact with the eyeball resulting in tearing and redness, are two other examples of conditions that are corrected by oculoplastic surgery.
Blepharoplasty
Why see an ophthalmologist for these conditions? Many people, when seeking plastic surgical procedures, will first think of a plastic surgeon, not an ophthalmologist, especially if the surgery is purely for cosmetic reasons. Most general plastic surgeons, however, are not trained in managing the ocular complications that can occur from these operations. Removal of too much skin, for example, during a blepharoplasty can result in the inability to fully close the eye during normal blinking. This can result in a serious dry eye problem. Ophthalmologists are trained in managing dry eye problems, and are also familiar with the intricate anatomy surrounding the eyes. Thus, an ophthalmologist, trained in oculoplastic surgery, is fully qualified to manage all these situations.
