        Overall

        This Dr. and his staff falsified my medical records after harming me during oral surgery. The nurse falsified my medical records by erasing the post up call she made during which I reported injury to avoid filing of injury claim. Also this dr. appeared to have no liability insurance at the time I was injured in his practice.
