Oak Trace Care and Rehabilitation provides a caring, supportive environment for your loved one. We offer private and semi-private rooms for both men and women. We offer 24-hour licensed nursing care and a full staff of certified nursing aides to meet all our residents needs. Each resident has an individualized plan of care coordinated with the resident, family, nursing, social services, and dietary services. The result is a comprehensive, customized plan of care designed to promote the residents personal well-being. We are proud to offer our residents an intensive Rehabilitation Program with a focus on Physical Therapy, Occupational Therapy, and Speech-Language Therapy. We use the latest technology to maximize our residents unique abilities to reach individual rehabilitation goals. We recognize that nursing homes and assisted living facilities are at high risk for contracting and spreading infectious diseases including COVID-19 (Coronavirus Disease 2019). Our utmost concern is for the health and well-being of each of our residents and to ensure our staff is healthy and able to continue to care for residents.
Immunizations
We would love for your family pet to come to visit. Please remember to clean up after them and take the proper safety measures at all times. All pets are to be kept on a leash and should have all current vaccinations.
Physical therapy works toward specific goals of achieving the residents’ highest functional level of independence. We focus on the identification, prevention, and rehabilitation of individuals with physical impairment and pain. Following evaluation, the therapists develop an individualized treatment program.
Occupational Therapy
Occupational therapy assists our residents in adapting to their social and physical environment by mastering tasks essential for daily living. Our therapists may work with residents who have debilitating arthritic conditions or difficulty bathing, dressing and other activities of daily living. They assist residents with impaired vision and/or hearing to function well in their environment.
Our social services team assists both residents and their families during transitional periods. They also provide essential information, manage requests and concerns, and help in the care and discharge planning for each resident. They can arrange professional services for Dental, Vision, Podiatry, Audiology, Power of Attorney, Dementia assessment, Psychosocial and Psychiatric Evaluation, and Individual therapy.
