Tiara Tolbert, MSN, CRNP, is a certified nurse practitioner with Hoffman and Associates OB/GYN, an all-female practice affiliated with Mercy Medical Center. She offers individualized compassionate care for women from puberty through menopause. She provides services such as prenatal exams, nutritional care and health management. Tiara offers expectant mothers care throughout their pregnancy and works in collaboration with Teresa Hoffman, M.D., and the physician team to care for mother and baby throughout labor and delivery.

Thyroid
Not necessarily. There are many causes of irregular bleeding, including thyroid disease and certain diseases of the uterus. It is very normal for your cycles to become heavier, closer together (21 to 24 days when previously 28 to 30 days) with more cramping and clotting. Frequently, it can look different than you are used to as you get older (starting late 30s to late 40s). We can evaluate symptoms and reasons for irregular bleeding with blood tests and an ultrasound.

Ultrasound
We will plan to see you in the office roughly every 4 weeks until you get to 28 weeks, then every 2-3 weeks until 37 weeks, then every week. You will have your blood drawn at the 8-10 week visit, at 15-20 weeks for second trimester markers, and around 28 weeks to check for gestational diabetes and anemia. You will have an ultrasound at 18-20 weeks to make sure the baby is developing normally at a radiology center. Usually they will be able to tell you the sex of the baby if you are interested. We do not do that in our office. If everything looks fine, that may be the only formal ultrasound during the pregnancy or gynecologic conditions when necessary.
