Laser Eye Surgery
In the United States, there are only a few hundred ASOPRS members, surgeons who have devoted themselves to the sub-specialty of oculofacial plastic and reconstructive surgery. After medical school and internship, ASOPRS surgeons complete four years of accredited eye surgery training and become board certified ophthalmologists. Then, after two years of extensive accredited oculofacial plastic and reconstructive surgery fellowship training, they must pass the qualifying written and oral examinations, and also have an original scientific investigation and thesis critically reviewed and accepted. The surgeon is then eligible to be considered by their peers for membership in ASOPRS. This indicates that your surgeon is not only a board certified ophthalmologist, but also has had extensive training and accreditation in oculofacial plastic and reconstructive surgery. It is important to recognize that even following such training, it takes years of experience to safely perform surgical procedures on the delicate tissues around the eyes and face. When you are ready for surgery, you will be in experienced hands.
Cataracts
This condition can be either unilateral or bilateral and reflects a change in the levator muscle (the principal upper eyelid elevator muscle) or much less frequently is the result of deficient nerve supply to this muscle. In adults, aging is the principal factor behind the stretching of the levataor muscle which can either become weakened or detached (levator aponeurosis dehiscence). However, ptosis may also follow trauma, neuromuscular disorders such as myesthenia gravis or Horner's syndrome, or cataract surgery.

Ectropion is an outwardly turned (everted) or sagging eyelid - usually the lower lid. Normally, the upper and lower eyelids close tightly, protecting the eye from damage and preventing tear evaporation. If the edge of one eyelid turns outward, the two eyelids cannot meet properly and tears are not spread over the eyeball. The wet, inner conjunctival surface is exposed and visible. This may lead to crusting of the eyelid, mucous discharge, pain, irritation, burning, a gritty, sandy feeling, and redness of the eyelid and conjunctiva with light sensitivity. The sagging leaves the eye exposed and dry, and as a result excessive tearing is common with ectropion. More severe and prolonged cases can also result in scar-induced closure of the punctum (the opening of the tear drainage system) exacerbating the tearing. Ectropion can cause serious ocular inflammation, resulting in damage to the eye, particularly the cornea - with potential ulceration. Ectropion can be diagnosed on routing eye exam. Special tests are usually not required.

Although the cause of blepharospasm is unknown, its manifestations arise from irritation of the facial nerve at the base of the skull. Uncommonly, this irritation may be the result of an abnormal blood vessel pulsating against the facial nerve. When the facial nerve is irritated, it causes the facial muscles to involuntary contract and go into spasm. Fewer than 1% of cases are caused by tumor-induced facial nerve irritation. This may require and MRI to document this uncommon cause. Otherwise, the diagnosis is made during clinical examination when the extent, severity, and frequency of the involuntary squeezing can be assessed. Blepharospasm, hemifacial spasm, and Meige's syndrome ordinarily require no further diagnostic testing.

Radiation Therapy
Capillary hemangiiomas are vascular lesions that may be present at birth or appear within the first six months of life. They have a predilictiion for the upper inner orbit, but may also occur in other orbital locations or in eyelid skin or soft tissue. These defects generally present as a bright red elevated area of the skin (strawberry hemangioima). They can also occur on other portions of the face, neck, or back. Some are quite small, while others can be so large that they deform the eyelid and cover the eye. At times this mass enlarges when the baby cries as its vascular channels become engorged. Characteristically, capillary hemangiomas enlarge from 6 months of age to 2 years of age, followed by a period of stagnation and subsequent partial or complete involution (spontaneous diminution in size). Those that do dissipate significantly require no treatment. At times these benign vascular tumors may hemorrhage. This blood accumulation gradually subsides and usually requires no treatment. Yet, treatment is indicated if the hemangioma is large - preventing the child from developing formed vision, causing amblyopia (lazy eye), or significant astigmatism. Treatment is usually centered around steroid injections into the hemangioma (intralesional steroids). Systemic steroids, surgical debulking, and radiation therapy have a negligible role as they provide limited benefit and high risk.

Reconstructive Surgery
The correct surgical method involves excisional biopsy of the tumor with frozen and paraffin section control - followed by immediate reconstructive surgery - all performed during 1 surgical procedure. A frozen section involves immediate evaluation of the tumor specimen during the surgery. In this regard the pathologist confirms the diagnosis and, if malignant, evaluates the margin to ensure that the surgical margin is completely free of tumor. Frozen section diagnosis is 99% accurate. The paraffin section is a confirmatory step performed by the pathologist over the subsequent 1 - 2 days to confirm whether the margins are definitively free of tumor and the frozen section diagnosis was accurate. Paraffin sections are practically 100% accurate. During surgery, Dr. Kohn does not move from the tumor excision stage of the procedure to the reconstruction stage until he has frozen section corroboration that the surgical margins are completely free of tumor.

Dr. Kohn's subsequent research has included further codification of the clinical findings, along with establishing the surgical approach to correcting the Kohn - Romano Syndrome (blepharophimosis syndrome). His additional clinical research focused on the surgical treatment of eyelid retraction (often seen in Graves' disease/hyperthyroidism), ectropion, and ocular pemphigoid, along with the evaluation and management of rhino-orbital mucormycosis (a serious orbital and sinus fungal infection). Dr. Kohn was also selected as one of the fifty sub-investigators across the U.S. to do the initial clinical research on botulinum toxin (Botox). After over 10,000 injections, their work led to the initial FDA (U.S. Food and Drug Administration) approval of this significant and widely useful medication.
Brow Lift
Brow lift for left acquired facial nerve palsy (Bell's Palsy). Left lower eyelid ectropion (sagging) was also corrected.
Eyelid Surgery
Eyelid retraction repair. Eyelid surgery to adjust the position of a retracted upper or lower eyelid can restore function by normalizing eyelid closure and also improve eyelid appearance. This is accomplished through quantified recession of the eyelid retractor muscles. In this regard, Dr. Kohn developed the well accepted and commonly used surgical procedure for this condition -
Plastic Surgery
With a keen interest in plastic surgery, Dr. Kohn followed his residency with two prestigious fellowships in oculofacial plastic surgery. The first was under the direction of Alston Callahan, M.D. at the University of Alabama Eye Foundation Hospital. Dr. Callahan was one of the three founders of the subspecialty of oculofacial plastic surgery. This fellowship provided fellows with perhaps the most extensive and hands-on experience available in the United States. Dr. Kohn then moved on to a second oculofacial plastic surgical fellowship. This program was at Harvard Medical School under the direction of Charles Beyer, M.D., Chief of the Oculoplastic Division at Harvard. This program was centered at the Massachusetts Eye and Ear Infirmary with additional experience based at Massachusetts General Hospital. These institutions are considered by many to be the best hospitals of their kind in the world.

Blepharoplasty
Surgery to improve the appearance of the eyelids is called blepharoplasty. The goal of blepharoplasty is to give the eyes a more youthful look by removing excess skin, bulging fat, and lax muscle from the upper and lower lids. If the sagging skin obstructs peripheral vision, blepharoplasty can eliminate the obstruction and expand the visual field.
Laser Resurfacing
Laser resurfacing may be performed on the whole face or in specific regions and may be performed in conjunction with other cosmetic and/or reconstructive procedures on the face or eyelids. Light energy is used to penetrate to a very precise, controlled depth, making the procedure safe and predictable, reducing the risk of resultant scarring or pigment problems. The light energy frequency of the various lasers, along with their multiple surgeon-controlled settings selectively achieves the results necessary to accomplish the desired result. All treatment modalities involve the sequential pass of the laser over the area undergoing treatment. The number of passes is dictated by the laser system in use and by the type and extent of the condition undergoing treatment. All laser surgery is performed in an outpatient setting. The type of laser and the extent of treatment determines whether local or general anesthesia is applicable.

Facial Implants
Fat introduction through repositioning/transfer or injection, along with placement of facial implants are not usually performed as an isolated surgery. Rather, they are often used as an adjunct during other cosmetic or reconstructive procedures.

Skin Cancer
The outer layer of skin is called the epidermis. Epidermal cells include the squamous cells, round basal cells, and pigment producing melanocytes. The dermis is the deeper layer of skin and contains the hair follicles, oil and sweat glands, and blood vessels. Skin cancers can arise from any of these skin cells. A biopsy is usually required to comfirm the diagnosis of skin cancer.

Fat in the lower lid can be removed or repositioned through an incision hidden on the inner surface of the lid (transconjunctival blepharoplasty). Laser resurfacing or a chemical peel can be performed at the same time if desired, to smooth and tighten the lower lid skin.
