An x-ray should be taken to ensure that there is not a fracture. X-rays also can be used to examine the joints and bone density, ruling out arthritis (particularly rheumatoid arthritis and osteoarthritis).
Plantar Fasciitis
Gait abnormalities can cause inflammation of the structures attached to the heel bone, resulting in heel pain. Inflammation of the plantar fascia, called plantar fasciitis, is the most common cause of heel pain.
Cryotherapy
Your doctor will likely suggest you apply the medicine regularly at home, followed by occasional visits to the doctor’s office. Your doctor may pare away part of the wart or use freezing treatment (cryotherapy). Studies show that salicylic acid is more effective when combined with freezing.
Medical Weight Loss
Many different treatments have been effective, and although it typically takes six to eighteen months to find a favorable resolution, plantar fasciitis has a generally good long-term prognosis. The mainstays of treatment are stretching the Achilles tendon and plantar fascia, resting, keeping off the foot as much as possible, discontinuing aggravating activity, cold compression therapy, contrast bath therapy, weight loss, arch support and heel lifts, and taping. To relieve pain and inflammation, nonsteroidal anti-inflammatory drugs (NSAIDs) such as aspirin and ibuprofen are often used but are of very limited benefit.One small, placebo-controlled study has shown a beneficial effect from glucosamine.

As with anyone facing any surgical procedure, those undergoing foot and ankle surgery require specific tests or examinations before surgery to improve a successful surgical outcome. Prior to surgery, the podiatric physician will review your medical history and medical conditions. Specific diseases, illnesses, allergies, and current medications need to be evaluated. Other tests that help evaluate your health status may be ordered by the podiatric physician, such as blood studies, urinalysis, EKG, X-rays, a blood flow study (to better evaluate the circulatory status of the foot/legs), and a biomechanical examination. A consultation with another medical specialist may be advised by a podiatric physician, depending on your test results or a specific medical condition.

Local injection of corticosteroids often gives temporary or permanent relief, but may be painful, if not combined with a local anesthetic and injected slowly with a small-diameter needle.Recurrence rates may be lower if injection is performed under ultrasound guidance.
MRI
An MRI scan (magnetic resonance imaging) is used to ensure that the compression is not caused by a tumor in the foot. An MRI also determines the size of the neuroma and whether the syndrome should be treated conservatively or aggressively. If surgery is indicated, the podiatrist can determine how much of the nerve must be resected. This is important, because different surgical techniques can be used, depending on the size and the position of the neuroma. Because MRIs are expensive, some insurance companies are reluctant to pay for them. If the podiatrist believes an MRI is necessary, he or she can persuade the insurance company to pay for it by presenting data to support the recommendation.

The diagnosis of Neuromas is made by a physical exam and a thorough history of the patient\’s complaint. Conditions that mimic the pain associated with Neuromas are stress fracture of the metatarsals, inflammation of the tendons in the bottom of the toes, arthritis of the joint between the metatarsal bone and the toe, or nerve compression or nerve damage further up on the foot, ankle, knee, hip, or back. X-rays are generally taken to rule out a possible stress fracture or arthritis. Because nerve tissue is not seen on an x-ray, the x-ray will not show the neuroma. A skilled foot specialist will be able to actually feel the neuroma on his exam of the foot. Special studies such as MRI, CT Scan, and nerve conduction studies have little value in the diagnosis of a neuroma. Additionally, these studies can be very expensive and generally the results do not alter the doctor\’s treatment plan. If the doctor on his exam cannot feel the neuroma, and if the patient\’s symptoms are not what is commonly seen, then nerve compression at another level should be suspected. In this instance, one area to be examined is the ankle.

Certification and advancement. There are a number of certifying boards for the podiatric specialties of orthopedics, primary medicine, and surgery. Certification has requirements beyond licensure. Each board requires advanced training, the completion of written and oral examinations, and experience as a practicing podiatrist. Most managed-care organizations prefer board-certified podiatrists.
Sciatica
Shin pain may also be the result of inflammation of connective tissue such as periosteum (periostitis). The pain may be caused by a stress fracture in the bone or some other problem like osteosarcoma. Pain in the lower leg may also be referred from a distant area of the body, such as pressure on the sciatic nerve (sciatica) which lies in the posterior thigh.

Metatarsal Surgery
There are numerous over the counter treatments for corns and calluses. Some of these remedies have an acid in them that burn the callous off. Care should be taken when using these medications. If used incorrectly they can cause a chemical burn to the skin. Additionally these remedies are only temporary because the source of the pressure has not been alleviated. Professional treatment consists of using a special shoe insert called a functional orthotic that corrects foot function. In certain instances surgery may be recommended. Surgery is directed at correcting the alignment of the offending bone. Cutting out the callous will only make the condition worse if the underling boney problem is not corrected. Metatarsal surgery is discussed in another section.
Bunions
Bunions are painful swellings that develop most often on the inner side of the foot near the base of the first toe (hallux). Less frequently, bunions occur at the base of the fifth toe (called a “tailor’s bunion”).
Gangrene
If an ingrown nail is left untreated, there exists a high risk of dangerous infection. When the skin around the nail gets infected, it begins to swell up and put even more pressure against the nail. Ingrown nails can produce a spear shaped wedge of nail on the lateral side of the toe which will progressively become more embedded into the toe tissue as the nail grows forward. In the worst case, the swelling will begin putting sideways pressure on the nail, causing it to grow at a slant. This will cause both sides of the nail to eventually become ingrown and swollen. Eventually the swollen parts of the skin will begin to harden and fold over the nail. An untreated ingrown toenail will cause a person to walk with a limp, which over a long period of time may cause further pain and injury to the foot, leg and back owing to improper distribution of weight. Other non-direct effects of seriously ingrown nails include lack of exercise, constant and unrelenting pain and pressure which is often extreme and debilitating when weight has been on the foot for an extended period of time, the spread of infection, loss of appetite, inability to move around, and psychological effects (like anxiety, stress and feelings of despair). Amputation of the toe, foot or leg may be the final outcome if the infection is left untreated long enough for gangrene to set in. An untreated infection may also lead to a condition known as osteomyelitis, where the infection spreads to the bone of the infected digit. Once in the bone, the infection is more difficult to remove and may require the intravenous treatment of antibiotics. One should always consult a doctor when infection is present.

Ingrown toenails are known to physicians as onychocryptosis. They are common, painful conditions that occur when the skin on the side of a toenail grows over the edge of the nail, or when the nail grows into the skin.

When we walk or stand, our body weight is carried first on the heel and then on the ball of the foot, where the skin is thicker, to withstand the pressure. When this pressure becomes excessive, some areas of the skin thicken, in the form of corns and callus, as a protective response.

If left untreated, claw toes and hammertoes can become serious fixed deformities. In many cases, the inside of the shoe rubs against the bent toe joints, gradually causing calluses to form on the tops of the toes. The metatarsal heads support the body weight and pressure calluses and open sores (ulcer) can develop on the sole of the foot. When deformities reach this stage, the toes are fixed in a bent position and cannot be easily straightened. For this reason, it is important to obtain treatment for claw toes and hammertoes when they first begin to develop, before they become fixed.

Most calluses are symptoms of an underlying problem like a bony deformity, a particular style of walking, or inappropriate footwear. Some people have a natural tendency to form callus because of their skin type. Elderly people have less fatty tissue in their skin and this can lead to callus forming on the ball of the foot.

A bunion is a bulge or bump that develops on the side of the foot, near the base of the big toe. Physicians call this deformity hallux abducto valgus or HAV, meaning the hallux turns away (abducts) from the midline of the foot and twists so the inside edge touches the ground and the outside edge turns upward. Essentially, it is the big toe\’s tendency to point to the outside of the foot. The condition worsens over time, causing discomfort, pain, skin problems (e.g., corns and lesions), and difficulty walking.

In addition to ill-fitting and narrow shoes, other common causes for toe pain include skin conditions, such as dry, cracked skin, eczema, and psoriasis; fungal infections, such as athlete’s foot; bacterial infections; frostbite (i.e., damage to the skin and sometimes the underlying tissue caused by extreme cold); and fractures and bruises.

Most podiatrists have a solo practice, although more are forming group practices with other podiatrists or health practitioners. Physical Therapy
Most ankle strains and sprains heal in 2 to 6 weeks, with proper treatment. Severe injuries may take as long as 12 weeks to heal and may require physical therapy to restore full muscle balance and strength. Physical therapy may involve stretching the Achilles tendon, as well as coordination and speed exercises. During this time, the ankle may require taping or bracing to provide support until full function is regained.

Tendonosis: Overtime, a tendon injury may lead to a condition called tendonosis, which is a degeneration of the tendon. With tendonosis, the tendon becomes overstretched resulting in weakness or ankle instability.

The ankles support the entire body weight and ankle injuries are very common. Each year, approximately 2 million patients are treated for ankle sprains and strains and ankle fractures are one of the most common injuries treated by orthopedists and podiatrists.

Flat Feet
In most cases, toe deformities are caused by a muscle imbalance in the foot. Flatfeet (pes planus), traumatic injuries, and underlying diseases may increase the risk for claw toes and hammertoes. Diseases and conditions that increase the risk include neuromuscular diseases (e.g., multiple sclerosis, Charcot-Marie-Tooth disease, cerebral palsy), inflammatory diseases (e.g., rheumatoid arthritis, psoriasis), and diabetes.
Hammertoes
A hammer toe is a deformity of the second, third or fourth toes. In this condition, the toe is bent at the middle joint, so that it resembles a hammer. Initially, hammer toes are flexible and can be corrected with simple measures but, if left untreated, they can become fixed and require surgery.
Heel Spurs
To understand the cause of the pain one must understand the anatomy of the foot and some basic mechanics in the function of the foot. A thick ligament, called the plantar fascia, is attached into the bottom of the heel and fans out into the ball of the foot, attaching into the base of the toes. The plantar fascia is made of dense, fibrous connective tissue that will stretch very little. It acts something like a shock absorber. As the foot impacts the ground with each step, it flattens out lengthening the foot. This action pulls on the plantar fascia, which stretches slightly. When the heel comes off the ground the tension on the ligament is released. Anything that causes the foot to flatten excessively will cause the plantar fascia to stretch greater that it is accustom to doing. One consequence of this is the development of small tears where the ligament attaches into the heel bone. When these small tears occur, a very small amount of bleeding occurs and the tension of the plantar fascia on the heel bone produces a spur on the bottom of the heel to form. Pain experienced in the bottom of the heel is not produced by the presence of the spur. The pain is due to excessive tension of the plantar fascia as it tears from its attachment into the heel bone. Heel spur formation is secondary to the excessive pull of the plantar fascia where it attaches to the heel bone. Many people have heel spurs at the attachment of the plantar fascia with out having any symptoms or pain. There are some less common causes of heel pain but they are relatively uncommon.

Ingrown Toenails
If you trim your toenails too short, particularly on the sides of your big toes, you may set the stage for an ingrown toenail. Like many people, when you trim your toenails, you may taper the corners so that the nail curves with the shape of your toe. But this technique may encourage your toenail to grow into the skin of your toe. The sides of the nail curl down and dig into your skin. An ingrown toenail may also happen if you wear shoes that are too tight or too short.

High Impact Exercising—Are you doing the same high impact workout over and over? This repetitive motion may be putting a lot of stress on your feet which can result in corns, calluses, blisters, muscle cramps, acute knee and ankle injuries, plantar fasciitis, and metatarsalgia. Try adding some variety to your workouts such as alternating days between jogging and swimming. This will reduce the amount of stress you put on your feet and joints. Plus, it will keep your work out from getting boring.
Foot Orthotics
Treatment of heel pain generally occurs in stages. At the earliest sign of heel pain, aggressive calf muscle stretching should be started. Additionally, taking an oral anti-inflammatory medication and over-the- counter arch supports or heel cushions may be beneficial. The next phase of treatment might consist of continued calf muscle stretching exercises, cortisone injections and orthopedic taping of the foot to support the arch. If this treatment fails, or if there is reoccurrence of the heel pain, then functional foot orthotics might be considered. A functional orthotic is a device that is prescribed and fitted by your foot doctor, which fits in normal shoes like an arch support. Unlike an arch support, however the orthotic corrects abnormal pronation of the subtalar joint. Thus orthotics address the cause of the heel pain – abnormal pronation of the foot. Surgery to correct heel pain is generally only recommended if orthotic treatment has failed. There are some exceptions to this course of treatment and it is up to you and your doctor to determine the most appropriate course of treatment. Following surgical treatment to correct heel pain the patient will generally have to continue the use of orthotics. The surgery does not correct the cause of the heel pain. The surgery will illuminate the pain but the process that caused the pain will continue without the use of orthotics. If orthotics have been prescribed prior to surgery they generally do not have to be remade.
Plantar Fasciitis
In cases of chronic plantar fasciitis of at least 10 months duration, one recent study has shown high success rates with a stretch of the plantar fascia.
Podiatric Surgery
The type of foot surgery performed determines the length and kind of aftercare required to assure that your recovery from surgery is rapid and uneventful. The basics of all postoperative care involve to some degree each of the following: rest, ice, compression, and elevation. Bandages, splints, surgical shoes, casts, crutches, or canes may be necessary to improve and ensure a safe recovery after foot surgery. A satisfactory recovery can be hastened by carefully following instructions from your podiatric physician.
