Operated by Agency on Aging of South Central Connecticut, Connecticut Home Care Program for Elders, is a health care program that features community-based medical care services for adults above the age of 65. It provides health status assessment, case management and ongoing treatment monitoring services. Connecticut Home Care Program for Elders offers adult day care, meal home delivery and home health aide services. It includes skilled nursing visitation and mental health counseling services, as well as physical, occupational and speech therapies. The program provides companionship, emergency medical response and home chore assistance services.
