Orthotics –insoles custom fit to your foot by a podiatrist through molding the foot or molded directly to your foot are one of the most treatments for plantar fasciitis. So called custom orthotics from the Good Feet store or Dr. Scholl’s are not custom fit to your feet. They are prefabricated insoles, these insoles can be helpful, but they are NOT custom fit, thus they do not realign the foot structure.
Cryotherapy
There are two major types of plantar warts simple and mosaic. Simple warts are usually relatively small in size ~5mm and only have a single black dot. Mosaic warts are conglomerates of several warts and can be very large in size. Mosaic warts will have multiple black dots and may be very difficult to eradicate. There are many plantar wart treatments including caustic agents, excision, cryotherapy (freezing), laser destruction, injections, and stimulating the body’s immune system. Most of the topical agents available over the counter are ineffective. The OTC topicals are not strong enough to exfoliate the skin in order to destroy the cells infected by the virus. Regardless of the treatment, it is important to adequately debride or trim the thickened tissue.

Gout (also known as podagra when it involves the big toe)[1] is a medical condition usually characterized by recurrent attacks of acute inflammatory arthritis—a red, tender, hot, swollen joint. The metatarsal-phalangeal joint at the base of the big toe is the most commonly affected (approximately 50% of cases). However, it may also present as tophi, kidney stones, or urate nephropathy. It is caused by elevated levels of uric acid in the blood. The uric acid crystallizes, and the crystals deposit in joints, tendons, and surrounding tissues.

Foot Neurology or nerve problems typically present with nerve pain, contracture, numbness, reduced muscle strength or moisture changes in the feet. Nerve pain is an completely different type of pain than any other, it is characterized by burning, tingling, or pins and needle like sensations. In some cases there may be a combination of all the various types of symptoms or nerve problems. Various nerve conditions will have a similar presentation. Nerve conditions can be difficult for an experienced neurologist to identify the cause or even treat. A doctor should be consulted if you start to experience any symptoms consistent with neurological pain.

In lymphedema abnormal removal of interstitial fluid is caused by failure of the lymphatic system. This may be due to obstruction from, for example, pressure from a cancer or enlarged lymph nodes, destruction of lymph vessels by radiotherapy, or infiltration of the lymphatics by infection (such as elephantiasis). It is most commonly due to a failure of the pumping action of muscles due to immobility, most strikingly in conditions such as multiple sclerosis, or paraplegia. It has been suggested that the edema that occurs in some people following use of aspirin-like cyclo-oxygenase inhibitors such as ibuprofen or indomethacin may be due to inhibition of lymph heart action.

A Doppler ultrasound blood flow detector, commonly called Doppler Wand or Doppler probe, and a sphygmomanometer (blood pressure cuff) are usually needed. The blood pressure cuff is inflated proximal to the artery in question. Measured by the Doppler wand, the inflation continues until the pulse in the artery ceases. The blood pressure cuff is then slowly deflated. When the artery’s pulse is re-detected through the Doppler probe the pressure in the cuff at that moment indicates the systolic pressure of that artery.

Angiograms are invasive vascular studies, they use a die and x-ray or MRI to assess the circulation of blockages (thrombi). They are done by inserting a catheter into the patient’s artery in the leg with the help of a needle,the catheter is moved to the arteries in question, the die is released into the vessel and live x-ray images show blockages or bottle necks in the vessels.

Sciatica
Sciatica is a condition similar to that of peripheral neuropathy, except the symptoms will normally affect only one leg. An individual afflicted with sciatica will relate burning, tingling, or a sensation of pins and needles somewhere along the leg or foot. The area affected will correlate to the nerves being pinched in the lower back. Each sciatic nerve is made up of 5 branches from the spinal column each have specific areas on the legs, thighs, ankles, and feet they are responsible for.

Foot Wounds need to be managed in accordance with their presentation and condition. Initial inspection should determine if there is any infection contributing pressure, circulation, healing capacity, and overall health. Wounds tell us what need to be done. If there is no circulation- vascular surgery, pressure- remove the pressure, infection- treat the infection, wet wound- dry it, etc. As more and more wound care is being shifted to wound care nurses (WCN) or wound care teams, it seams every wounds treatment of course is the same- Santyl dressing QD. Getting wounds to heal requires treating the source be it pressure, infections, dietary concerns, circulation, and/or necrotic tissue.

Bunions are caused by a combination of genetic factors like foot type and environmental factors. As pressure is applied, the tissues surrounding the joint may become swollen and tender.
Gangrene
Peripheral Vascular disease (PVD) or Peripheral Arterial Disease (PAD) are characterized by decreased circulation to the extremities. There are many systemic conditions that can cause or facilitate blockages in the arteries serving blood to the legs, feet, and toes. Symptoms of peripheral vascular disease can include edema, discoloration, cold feet, pain, cramping, night cramps, night pain, claudication (cramping of calves when walking), ulcerations, dry gangrene, pain, and auto amputation. Typically we will find a combination of these symptoms. Peripheral vascular disease predisposes sufferers to infections, ulcerations, amputation, and death.

It is absolutely imperative that people have adequate circulation in order to heal wounds. If adequate circulation does not exist it may be necessary to perform vascular surgery to acquire sufficient blood flow. New techniques and vascular surgery have become less invasive and very productive.

Hammertoe Surgery
Surgical treatment is the only way to cure these deformities permanently. The corn is a symptom of the hammertoe and the hammertoe is a symptom of an unstable foot. Because the hammertoe is a structural deformity caused by bone, ligament, and tendons, they need to be altered to regain the proper alignment of the toe. This can be done different ways and depends on your podiatrist’s preferences. Typically, the healing time is about 3-4 weeks with hammertoe surgery, assuming there are no complications.

Corns
Interdigital soft corns occur between the toes and can occur in any of the interspaces. Usually one corn will form between the toes, however there can be a corn on the adjacent toe (communicating corn) as well. Interdigital corns can be very painful and come on quickly. Most are easy to trim by a skilled podiatrist. Interdigital soft corns are caused by bone spurs, arthritis, or enlarged toe joints creating excessive pressure between the toes. This pressure is exacerbated by tight fitting shoes.

It has been a success and with the increase in the number of diabetic Americas the benefits of this program have saved millions of dollars and many limbs. Having a good understanding of the way a foot functions and pressures in the foot is critical to preventing ulcers, infections, and amputations. Dr. Oehler has treated 1000’s of ulcers over his career medically and through the use of devices such as shoes, padding techniques, insole modifications, and specialized moldable devices for the toes. We have had great success with these modalities.
Calluses
Painful Toes are commonly caused by corns or calluses on toes, they are thickened skin deposits occurring in specific areas in response to pressure and shear force. In most cases, the pressure is caused by ill-fitting shoes, hammertoes, or arthritis. Corns may cause black areas, painful toes, infections, ulcers, and limit walking. If a corn has black spots (blood deposits), they are ulcers. These areas may or may not be painful. In people with diabetes and some other disorders, the nerves may not function properly, people may not have painful toes at all until the corn causes severe build up, an open wound or an infection. There are several different types of corns, they will be broken down into groups based on treatment and location.
Eczema
Chronic Venous Insufficiency (CVI) results from venous valve failure. Venous valves are required to provide efficient blood return from the lower extremities to the heart. Venous insufficiency often occurs in the veins of the legs. Itching (pruritus), swelling, lymphedema, and discoloration of the legs are common symptoms. The skin may react with eczema, local inflammation, discoloration, thickening, and an increased risk of ulcers and cellulitis.
Fungus
Nail fungus treatment can be one of the most frustrating and difficult infections to treat. These infections are resilient and resolve slowly. There are 5 treatment options available, oral medication, topical medication, laser treatments, debridement (maintenance trimming), and permanent removal of the nail.
Lesions
Plantar warts (verrucae) are caused by strains of the human papilloma virus (HPV). Plantar warts are skin lesions that usually occur on the bottom or plantar surface of the foot. Differentiating plantar warts from other skin lesions on the bottom of the foot can be difficult at times. Plantar warts occur in an estimated 7% – 10% of the population with a higher incidence in children.
Psoriasis
Hallux limitus and hallux rigidus are a result of decreased cartilage and or inflammation in the big toe joint. This is most commonly caused by over pronation in the mid foot leading to jamming of the 1st MTPJ or bunion formation. Metabolic conditions and autoimmune diseases like gout, pseudogout, RA, and psoriasis can also cause deterioration of the 1st MTPJ and other joints. If these diseases are treated appropriately deterioration can be minimized.

Hammertoes
Surgical treatment is the same as it is for the hammertoes in most cases. In years past Minimal incision surgery was performed by shaving off some of the bone responsible for the pressure. This surgery lost favor for various reasons.
Heel Spurs
Surgical treatment preferences depend on the doctor performing the surgery. Some doctors believe any heel spur must be removed and the plantar fascia must be severed. Other doctors believe just the plantar fascia must be severed. The reason this is important is because the healing times, post operative pain and post operative care are much different. I personally have family members who have just had the plantar fascia resected and feel that it worked perfectly with no complications. Endoscopic plantar fasciotomy is an easy less invasive way of severing the plantar fascia. The recovery is 2 –3 weeks and pain is minimal compared to the other more invasive procedures.

Surgical Hallux Limitus Treatment is the only way to eliminate or reduce pain in some cases. There are several types of surgery for the treatment of hallux rigidus and hallux limitus. The type of surgery is based on the the age of the patient, level of pain, activity level, and progression of the arthritis. Stage 1 hallux rigidus involves some loss of range of motion of the big toe joint or first MTPJ and is often treated conservatively with arch supports, prescription foot orthotics, NSAIDS, or steroid injections.

There are nerves in the heel area that can cause heel pain when entrapped and irritated by muscle and ligament.
