Colonoscopy
Prior to performing colonoscopy, the colon needs thorough cleansing so that there can be unobstructed view of the entire colon. Depending of the bowel preparation selected for you, you will need to be placed on clear liquids for one to two days prior to the procedure. This will increase the effectiveness of the bowel preparation. Similar to other procedures we perform, you will need to remain NPO after midnight. In most cases, you will be instructed not take your morning medications. Diabetic patients will need to receive special instructions on their insulin dosing prior to the procedure.

Upper endoscopic procedures (EGD) are relatively safe procedures. Major complications may include bleeding, perforation, aspiration pneumonia, or cardiac events. Large surveys indicate a risk of serious complications during diagnostic EGD of approximately 1 in 800 and a risk of death of approximately 1 in 5,000. In more seriously ill patients, such as those for whom EGD is performed to evaluate active bleeding at the time of the procedure, the risks are higher. In one such series, 1 in 200 patients had major complications of the procedure and the risk of death was 1 in 700. Because of its relative safety, EGD is usually substituted for X-ray in the evaluation of gastrointestinal symptoms in women who may be pregnant.
Immunizations
Additionally, people traveling to countries where the hepatitis B virus is known to be endemic (common) are at risk. These areas of the world include most developing countries, Asia, Africa, India, and the Middle East. Pregnant women can also pass the virus to their babies. Some people with HBV have no outward signs or symptoms. But others do experience "flu-like" symptoms, such as fatigue, loss of appetite, nausea and vomiting, fever, weakness and mild abdominal pain. Less common symptoms are dark urine and yellowing of the skin and eyes (jaundice). If patients have been infected with HBV for six months ore more, their doctors may feel they need treatment in order to avoid permanent liver damage. In addition to treatment, patients may also receive a vaccination for the prevention of hepatitis A.

Internal Medicine
Following medical school, Dr. Galati was an Intern and Resident in Internal Medicine at State University of New York Health Science Center-Brooklyn (formerly Downstate Medical Center)/Kings County Hospital Center, one of the premier teaching hospitals in the country. He remained an additional year in the department to assume responsibilities as the Chief Medical Resident in the Department of Medicine.
Gastroenterology
Wanting to pursue further training and expertise in Liver Disease/Transplant Medicine, Dr. Galati left New York City for Omaha, Nebraska. He performed his fellowship training in Gastroenterology, Hepatology, and Transplant Hepatology at University of Nebraska Medical Center. University of Nebraska Medical Center is a world renowned liver program, and today remains one of the finest in the country. During Dr. Galati’s training there, he participated in a number of research studies looking at the gastrointestinal complications of liver disease and cirrhosis.

Endoscopy
In most cases, endoscopy will be required. An upper endoscopy is a test that is performed where a flexible lighted instrument is passed through the mouth into the esophagus examining the esophagus, stomach, and the first portion of the small intestine looking to identify the source of bleeding. At the time of this upper endoscopy, depending on the source of bleeding, various techniques can be used to control the bleeding. If necessary, biopsies of the involved area can also be obtained at the time of the procedure.

Many people drink alcohol socially or to relieve stress. Mineral water and fruit juices can be substituted for alcoholic beverages at parties and social functions. Stress management techniques that do not involve alcohol can be adapted, such as regular exercise, yoga, or a meditation routine. The transition from social drinking to problem drinking can be very subtle, and sometimes people do not realize they have grown addicted to alcohol until they try to stop drinking and find that difficult. In the United States, approximately 9% of adult males and 4% of females are addicted to alcohol.

Hemochromatosis is diagnosed through a detailed medical history, physical examination, and blood work screening for various causes of elevated liver chemistries and chronic liver disease. Elevated levels of iron will be seen and the transferrin saturation will also be elevated as a marker of increased iron. Transferrin saturation higher than 45% would be considered significantly elevated. Additionally, the total iron binding capacity will be elevated greater than 50% in hereditary hemochromatosis. A liver biopsy in many cases is required. This will allow us to directly inspect the liver under a microscope staining for iron. In addition, the biopsy can be sent off for additional testing to measure the amount of iron in the liver.

Ultrasound
FibroScan® is a noninvasive diagnostic ultrasound-based device used to measure liver scarring, or fibrosis, caused by different liver diseases. Similar to a conventional ultrasound exam, outpatient FibroScan® testing is quick, painless and easy, and provides a non-surgical alternative to the traditional liver biopsy to assess liver damage.
MRI
Most hepatocellular carcinomas are first suspected based on the results of CAT scans, MRI, or ultrasound scans. Many times these scans are performed for other symptoms, such as abdominal pain. Blood alpha-fetoprotein, a blood tumor marker, is a useful lab test for the diagnosis of hepatocellular carcinoma. About 70% of patients with hepatocellular carcinoma have elevated blood alpha-fetoprotein concentrations; however, it is not specific for this condition. We often see normal alphafetoprotein values in the face of massive tumors in the liver. It is often measured as a part of screening in patients with chronic hepatitis B or chronic hepatitis C and cirrhosis. A rising blood alpha-fetoprotein concentration in someone with chronic liver disease suggests the development of hepatocellular carcinoma. Appropriate radiological scans should be done in such instances.
Radiology
At the point where the ascites is becoming problematic for the patient, we would recommend that a paracentesis be performed. Typically, the paracentesis is performed in radiology under ultrasound guidance. The skin on the abdomen is clean, and a small amount of lidocaine is used to deaden the skin. A needle is then inserted to the abdominal cavity where the fluid is present, and the fluid is drained. In the most circumstances, between one and 4 L may be removed. The fluid is sent to the laboratory for analysis, making sure there is no infection present. In more than 75% of the cases, patients can control their ascites with diet and diuretics. Large volume paracentesis is typically reserved for patients with more advanced liver disease, or those that are unresponsive to medical and dietary intervention. The presence of ascites in patients with cirrhosis would be an indication to consider liver transplantation

Chemotherapy
Hepatocellular carcinoma is curable by surgery only if the tumor is small. Liver transplantation may also be curative for relatively small tumors. Surgery or liver transplantation may not be possible in all cases, usually if the tumor is very large or has spread beyond the liver. If there is invasion of the major blood vessels of the liver with tumor, the hope for cure or transplant are reduced. For large tumors or cancer that has spread beyond the liver, chemotherapy, ligating (tying) or embolization (clotting) of the hepatic artery, alcohol injection into the tumor, or radiation may relieve symptoms and prolong life, but these procedures are not curative.

Gynecomastia
Abnormal biochemical function of the liver in cirrhosis can lead to several complications. The serum albumin concentration falls, which can lead to aggravation of ascites and edema. The metabolism of drugs can change requiring dose adjustments. In men, breast enlargement (gynecomastia) sometimes occurs because metabolism of estrogen in the liver is decreased. Sexual function in men will also become abnormal. Decreased production of blood clotting factors can lead to bleeding complications. Derangements in the metabolism of triglycerides, cholesterol and sugar can occur. In earlier stages, cirrhosis frequently can cause insulin resistance and diabetes mellitus. In later stages or in severe liver failure, blood glucose may be low because it cannot be synthesized from fats or proteins. Cirrhosis, especially in advanced cases, can cause profound abnormalities in the brain. In cirrhosis, some blood leaving the gut bypasses the liver as blood flow through the liver is decreased. Metabolism of components absorbed in the gut can also be decreased as liver cell function deteriorates. Both of these derangements can lead to hepatic encephalopathy as toxic metabolites, normally removed from the blood by the liver, can reach the brain. In its early stages, subtle mental changes such as poor concentration or the inability to construct simple objects occurs. In severe cases, hepatic encephalopathy can lead to stupor, coma, brain swelling and death.

Upper endoscopy is performed to evaluate problems related to the upper digestive tract, including the esophagus (the food pipe), stomach, and small intestine. Upper endoscopy (EGD) can provide valuable diagnostic information on the following disorders including upper abdominal distress, abdominal pain, nausea, and vomiting, especially that persisting after an appropriate trial of therapy, persistent esophageal reflux symptoms, also known as GERD, despite appropriate therapy, swallowing difficulties, persistent vomiting or nausea of unknown cause, X-ray findings of a tumor in the esophagus, stomach, or small intestine, X-ray findings of gastric or esophageal ulcer, evidence of a narrowing of the esophagus, to assess for malignancy, gastrointestinal bleeding, and to determine the presence of esophageal varicies in patients with cirrhosis and portal hypertension.
