Gastroesophageal reflux is a physical condition in which acid from the stomach flows backward up into the esophagus. People will experience heartburn symptoms when excessive amounts of acid reflux into the esophagus. Many describe heartburn as a feeling of burning discomfort, localized behind the breastbone, that moves up toward the neck and throat. Some even experience the bitter or sour taste of the acid in the back of the throat. The burning and pressure symptoms of heartburn can last for several hours and often worsen after eating food. All of us may have occasional heartburn. However, frequent heartburn (two or more times a week), food sticking, blood or weight loss may be associated with a more severe problem known as gastroesophageal reflux disease or GERD.

Colonoscopy
Colonoscopy enables your doctor to examine the lining of your colon (large intestine) for abnormalities. Your doctor will use a thin, flexible tube called a colonoscope, which has its own lens and light source, and will view the images on a video monitor. Please ask Dr. Bravo about anything you don’t understand.

Inflammatory bowel disease (IBD) refers to a chronic (long term/ lifelong) inflammation or irritation of the stomach, small intestine and/or colon (large bowel). Inflammatory bowel disease should not be confused with irritable bowel syndrome (IBS). There are two types or categories of inflammatory bowel disease, Ulcerative Colitis and Crohn’s disease. Patients can have either type but not both. Occasionally one type of inflammatory bowel disease is diagnosed and with further testing or time the disease may be re-diagnosed as the other type.

A healthy pregnancy, childbirth and baby are possible in women with CD. Well controlled CD does not increase the risk of miscarriages, stillbirths or congenital abnormalities. The goal during pregnancy is to prevent and treat flares aggressively to decrease any increased risk to the fetus during a flare.

Bladder Cancer
Kidney Stones
CD may cause symptoms outside the digestive tract including: inflammation of the eye (conjunctivitis, episcleritis, uveitis, iritis), inflammation of joints (arthritis), weakening of bones (osteoporosis), skin rashes (erythema nodosum, pyoderma gangrenosum), inflammation of the liver or bile ducts (primary sclerosing cholangitis), kidney stones, gallstones and in children, delayed growth or sexual development due to the use of steroids, malnutrition and malabsorption.
Ovarian Cancer
Anyone with a personal history of colorectal cancer or adenomas at any age, or cancer of endometrium (uterus) or ovary diagnosed before age 50. Persons who have had colorectal cancer or adenomas removed are at increased risk of developing additional adenomas or cancers. Women diagnosed with uterine or ovarian cancer before age 50 are at increased risk of colorectal cancer. These groups should be checked by colonoscopy at regular intervals, usually every 3 to 5 years. Woman with a personal history of breast cancer have only a very slight increase in risk of colorectal cancer.
Colon Cancer
Most colon and rectal cancers originate from benign wart-like growths on the inner lining of the colon or rectum called polyps. Not all polyps have the potential to transform into cancer. Those that do have the potential are called adenomas. It takes more than 10 years in most cases for an adenoma to develop into cancer. This is why some colon cancer prevention tests are effective even if done at 10-year intervals. This 10-year interval is too long, in some cases, such as in persons with ulcerative colitis or Crohn’s colitis, and in persons with a strong family history of colorectal cancer or adenomas.
Endoscopy
Upper endoscopy lets your doctor examine the lining of the upper part of your gastrointestinal tract, which includes the esophagus, stomach and duodenum (first portion of the small intestine). Your doctor will use a thin, flexible tube called an endoscope, which has its own lens and light source, and will view the images on a video monitor. You might hear your doctor or other medical staff refer to upper endoscopy as upper GI endoscopy, esophagogastroduodenoscopy, EGD or panendoscopy. Please ask Dr. Bravo about anything you don’t understand.

Stress Management
Some patients will not tolerate milk products (containing the milk sugar – lactose) or other concentrated sugars such as fructose (corn syrup). Taking multivitamins and speaking to a dietician also can be of great benefit. Exercise and stress management can decrease flares and help moderate the disease. Smoking should be stopped by all who have CD or are at increased risk of developing CD. A group of medications, known as NSAIDS (aspirin, ibuprofen, naproxen), should also be avoided as they can worsen CD in some patients.

CD affects many ethnicities, with the highest prevalence in Caucasians. Infants and children may have celiac disease, but CD is more commonly diagnosed in adulthood, and people can be diagnosed even in their seventies or eighties. Female patients are more likely to be diagnosed with celiac disease than male patients. Individuals that have type 1 diabetes, thyroid disorders, or relatives with CD are at greater risk for developing CD.

Ultrasound
Abdominal Ultrasound: this is the safest, simplest and most readily available test to diagnose gallstones. Sonar waves from a probe are passed over the abdomen to visualize the gallbladder and detect the presence of stones. This test is not an x-ray and can be safely performed during pregnancy.
MRI
Additionally, because the capsule contains a small magnet, you should not undergo an MRI study within 30 days of having a Bravo pH test.
Radiology
CT scan: This radiology test takes multiple cross-sectional pictures of the body. It is not generally performed to make a diagnosis of diverticulosis, but this type of exam, when done for other reasons, may identify diverticula.
X-Rays
Nuclear Medicine
Nuclear medicine bleeding scans (a type of radiology test) can also be used to identify the site of the diverticular bleeding if endoscopy does not or if bleeding is very severe. Occasionally, angiography (injection of dye into the abdominal blood vessels by a radiologist) is needed to identify and treat diverticular bleeding. In rare cases in which endoscopic or radiologic management fails to control the bleeding, surgery may be necessary to remove the part of the colon that is bleeding.
Computed Tomography
The diagnosis of acute pancreatitis is made by a combination of symptoms, physical exam findings, and laboratory tests including amylase and lipase. If the diagnosis is uncertain, abdominal imaging studies such as a computed tomography (CT) scan may also be necessary.
Chemotherapy
Cancer of the pancreas is really only curable if it is found in the early stages. Surgery, radiation and chemotherapy are possible treatment options. Surgery may be done to remove all or part of the pancreas and surrounding tissues if needed. Radiation therapy can be used to damage the cancer cells and prevent them from growing. Radiation maybe used in certain trials after surgery to help kill any remaining cancer cells. Chemotherapy will not cure pancreatic cancer but may have some effect on slowing the rate of progression of the tumor or to improve the patient’s quality of life. Many new drugs are being investigated for chemotherapy of pancreatic cancer and patients with this disease may have an opportunity to participate in one of the research trials for chemotherapeutic treatment of pancreatic cancer.
Radiation Therapy
Chronic diarrhea is classified as fatty or malabsorption, inflammatory or most commonly watery. Chronic bloody diarrhea may be due to inflammatory bowel disease (IBD), which is ulcerative colitis or Crohn’s disease. Other less common causes include ischemia of the gut, infections, radiation therapy and colon cancer or polyps. Infections leading to chronic diarrhea are uncommon, with the exception of parasites.

Once a diagnosis is made, treatment can proceed for that condition. Sometimes medications are used to reduce inflammation or affect the function of an organ, thereby relieving pain. For example, ulcers can be treated by taking medications that reduce stomach acid secretion. As the ulcer heals, pain is reduced. Sometimes an operation is needed to correct a problem. For example, pain due to cholecystitis (inflammation of the gall bladder) is usually treated by removal of the gall bladder (cholecystectomy).Occasionally, pain must be treated with drugs that reduce pain (analgesics). Simple analgesics like aspirin and ibuprofen should not be used ordinarily for undiagnosed abdominal pain because they may cause other problems, such as ulcers. Narcotic drugs are sometimes prescribed by doctors for abdominal pain, but their use can lead to constipation and other abdominal symptoms.
Lesions
Colon cancer. A key advantage of this technique is that it allows both imaging of abnormal findings and also therapy or removal of these lesions during the same examination. This procedure is particularly helpful for identification and removal of precancerous polyps.

We do not yet know an exact cause of CD. Similar to other chronic inflammatory disorders (e.g. asthma, rheumatoid arthritis, psoriasis, multiple sclerosis) there is a complex interaction between an individual’s genetic make-up, their immune system and foreign substances in the environment (including dietary factors and microbes living in our gut) that are responsible for the chronic uncontrolled inflammation in CD. The impact of the various factors (genes, immune system and environment) are different for each individual accounting for the broad spectrum of how patients present with inflammation in different segments of the intestine and in various degrees of severity. Unfortunately, it appears as if once this inflammatory process is set in motion it cannot be regulated or reversed by the body itself.
