Nutritional Counseling
Protein. Protein is critical in the diet as a source of amino acids for building muscles and active proteins (enzymes) in the body. Proteins are found in meat, eggs, milk, beans, nuts and other sources. Consult books on diet and nutrition for the dietary quality of each protein source. Many of my patients have high cholesterol and/or Diabetes; these patients need special guidance and monitoring of protein intake in an anti-aging regiment.
Primary Care
Most Osteopathic physicians tend to go into primary care (general practice, family practice, etc.). A fair percentage become specialists in every field from cardiology to neurosurgery. The reason that a high percentage of us go into primary care is that we are taught an integrated approach to health. Typical allopathic (MD's) training tends to sectionalize medicine into a disease model. This encourages their graduates to go into sectional (specialty) fields. There is no wrong or right with these different approaches, and, in fact, it affords Americans with a balanced choice of physicians.

My name is Alan Zend, and I've been in Family Practice since 1976. I grew up in New York City where I graduated from the City College of New York with a Bachelor of Science in 1971. I went to medical school at Des Moines University, College of Osteopathic Medicine and Surgery in Iowa, and I did my internship at Good Samaritan Hospital in Cincinnati, Ohio.

Historians attribute much of this increase to public health advances, including general access to clean water, fresh food, better pre- and post-natal care, immunizations, and improvements in sanitation. Early in this century, developed countries also made great strides in eradicating infectious diseases like smallpox, yellow fever, tuberculosis, and deadly pneumonia, which had traditionally ravaged America's cradles and schoolrooms. In 1900 only 19 percent of individuals who died were over age 65, while today 72 percent of all deaths occur in those over 65. However, more recent advances in technology, wider access to health care, and general prosperity have allowed even older Americans to live longer. As a result, those over age 75--or the "old-old"--now represent the fastest growing segment of our population.

Macular Degeneration
Age-related macular degeneration (AMD) is the leading cause of blind registration in the developed world, and yet its pathogenesis remains poorly understood. Oxidative stress, which refers to cellular damage caused by reactive oxygen intermediates (ROI), has been implicated in many disease processes, especially age-related disorders. ROIs include free radicals, hydrogen peroxide, and singlet oxygen, and they are often the byproducts of oxygen metabolism. The retina is particularly susceptible to oxidative stress because of its high consumption of oxygen, its high proportion of polyunsaturated fatty acids, and its exposure to visible light.
Cataracts
Once this binding has occurred the protein becomes impaired and is unable to perform as efficiently. Living a longer life is going to lead to the increased possibility of oxygen meeting glucose and protein and known cross-linking disorders include senile cataract and the appearance of tough, leathery and yellow skin.

Cervical Cancer
Folic acid is an extremely important vitamin that is involved in many activities in the body. Because folic acid is necessary for nerve formation and regulation and especially, nerve formation in the fetus, women, in their child bearing years, whether pregnant or not, should routinely take supplemental folic acid (400 mcg daily) to help prevent serious birth defects such as spina bifida and other neuronal disorders. Folic acid helps in the formation of red blood cells, the production of energy, the formation of white blood cells and is crucial for the synthesis of DNA, which is the genetic code of your body. Adequate intake of folic acid has been shown to be helpful in treating some anxiety disorders and depression. Women with adequate folic acid levels in their bodies had a lesser incidence of cervical dysplasia (abnormal cells in the cervix) which can be a precursor to cervical cancer. Folic acid may be found in many fruits and vegetables. Good sources of folic acid are navy beans, pinto beans, asparagus, broccoli, okra, spinach and brussel sprouts. We recommend 400 mcg to 800 mcg of folic acid daily to be obtained from your diet and from supplementation.
Colon Cancer
Complicating these results, however, was the vexing finding that a drink or two daily also increased the risk for breast cancer in the nurses and the risk for colon cancer in the male health professionals. With further analysis, Rimm and his colleagues found that moderate drinkers in the study who took daily supplements of about 400 to 800 micrograms of folic acid canceled out this elevated cancer risk. Which brings up another major controversy in America's quest to stay youthful: Can supplements help? "I take a multiple vitamin as do most of my colleagues," says Rimm. "While supplements can't replace a healthy diet, it is hard to get necessary amounts of some vitamins in natural form."Taking supplements of folic acid, a critical B vitamin, may be especially beneficial since it has not only been shown to lower cancer risk but is also associated with decreased risk for cardiovascular disease in both men and women; taking at least 100 ius of the antioxidant vitamin E also lowered risk for heart disease--up to 40 percent in both male and female participants.
Prostate Cancer
Below is the standard drill on Vitamin E. However, well controlled studies have failed to show any benefit from Vitamin E in regards to preventing cancers or heart disease. The only somewhat consistent finding is that it has a small, but identifiable, statistical increase in strokes. There's even been a recent study associating Vitamin E and prostate cancer. I'll leave this rhetoric below, but my bottom line is that I no longer take it.
Lung Cancer
Good dietary sources of beta-carotene are cantaloupe, spinach, various dark green leafy vegetables, romaine lettuce and apricots. Beta-carotene, a precursor to vitamin A, has been known for its antioxidant effects in the prevention of many cancers and heart disease. There was a Finnish study a few years ago that showed an increased risk of lung cancer in smokers who used beta-carotene. There is, therefore, some debate over the effectiveness of beta-carotene alone, in our diets. Recent research has shown that foods rich in beta-carotene are also rich in lycopene, lutein, zeaxantin and alpha carotene, all strong disease fighting carotenoids. What these amount to is good health for your eyes, especially in regards to preventing Macular Degeneration (the leading cause of adult blindness). A commonly recommended dose is 20,000 IU of beta-carotene per day, to be obtained primarily from the food you eat and secondarily, from supplementation.

Multiple Sclerosis
The fatty covering that protects nerve fibers in your body is called the myelin sheath. Vitamin B12 is necessary in the production of the myelin sheath. Severe deficiencies of Vitamin B12 may cause a deterioration of the myelin sheath, which is evident in patients with multiple sclerosis. Low levels of Vitamin B12 may cause increased homocysteine (a substance that is formed from protein metabolism) levels which, in turn, may cause more clotting in the arterial walls. Vitamin B12 is important in the production of red blood cells. Vitamin B12 can be found in ham, cooked oysters, crab, tuna, salmon, clams and herring. We recommend 400 mcg of Vitamin B12 daily to be obtained primarily from you diet.

In her most recent book, Strong Women, Strong Bones, she points out that by as young as 35, women begin to lose up to 1 percent of their bone mass each year; for more than 28 million Americans (including two million men), this loss of bone mass leads to osteoporosis and, in many cases, premature death. To prevent crippling disability and loss of productive daily life, Nelson lays out an easy-to-follow program of exercise and nutrition based on years of lab research. She has tailored these programs for nursing homes, community centers, and retirement communities to encourage even the most fragile seniors to start resistance training. The results have been astounding--from wheelchair-bound residents who can walk again to less disabled seniors who are once more able to golf, bike, water ski, or just bring in their own groceries. "Furthermore," Nelson adds, "the physical benefits are often accompanied by mental health gains, including increased self-esteem and self- confidence, decreased depression, and improved sleep."

Skin Cancer
Para-Aminobenzoic Acid (PABA) is a primary ingredient in folic acid. It also helps in the metabolism of Vitamin B5. PABA is a powerful antioxidant that helps prevent sunburn and skin cancer. PABA helps with red blood cell formation, assists in protein metabolism and is integral in maintaining proper intestinal health. Good sources of PABA are various organ meats such as kidneys and liver, whole grains, spinach, molasses and mushrooms. We recommend 50 mg of PABA daily, to be obtained from your diet and from supplementation.

Ingrown Toenails
My practice has the typical diverse nature of a general practice. I treat every thing from ingrown toenails to migraine headaches. We see children and patients pushing into their 90's. Having had the additional training as an Osteopath, and my self-education in scientifically proven "alternative" forms of medicine, have given my traditional Western medical practice new and exciting dimensions. The Osteopathic training has allowed me to further aid those patients with musculo-skeletal problems, and the "alternative" approaches have allowed me to avoid using the phrase "learn to live with it" many times.
