Achalasia usually first occurs in mid-life, although congenital and familial cases have been described. The symptoms have a slow onset and progress gradually. As a result, many people delay seeking medical attention until symptoms are advanced. The major symptom is difficulty swallowing: about 90 percent of affected people have trouble swallowing food and 85 percent of affected people have trouble swallowing liquids. Other symptoms include chest pain, regurgitation of swallowed food and liquid, heartburn, difficulty burping, a sensation of fullness or a lump in the throat, hiccups, and weight loss.

— Irritable bowel syndrome (IBS) is a chronic condition of the gastrointestinal tract. Its main symptoms are abdominal pain and altered bowel habits which have no identifiable cause.

Lung and throat problems — Some patients reflux acid up to the throat, which results in inflammation of the vocal cords, producing sore throats or a hoarse voice. When acid is refluxed into the throat, it can get into the lungs and cause an aspiration pneumonia or asthma symptoms. Chronic acid reflux into the lungs may eventually produce permanent lung damage, called pulmonary fibrosis or bronchiectasis.
Immunizations
Patients with celiac disease should be tested for nutritional deficiencies that may result from malabsorption. Common tests include a blood count to detect anemia from iron, folic acid, or vitamin B12 deficiency, and tests to detect calcium or vitamin K deficiency. Many doctors recommend that patients with celiac disease undergo specific testing for bone loss. One method involves using a DEXA (dual energy x-ray absorptiometry) scan, a painless imaging test that determines the density of bone. Those who have developed significant bone loss may require specific treatment. Vaccination against pneumonia (Pneumovax) has also been recommended.

Glaucoma
Eye inflammation — Inflammation of the eyes occurs in up to 5 percent of patients with Crohn's disease. This inflammation often responds to the drugs used to treat Crohn's disease, but careful monitoring is necessary to prevent complications such as glaucoma.
Cataracts
Corticosteroids — Corticosteroids are usually the most problematic drugs for patients since they have many side-effects. Increased appetite, weight gain, acne, fluid retention, tremulousness, mood swings, and difficulty sleeping are very common. Many other side-effects occur in patients who take corticosteroids for long periods of time, particularly if high doses are used. These include diabetes, thinning of the skin, easy bruising, a "cushingoid" appearance (widening of the face and a hump in the back), osteoporosis, body hair growth, cataracts, high blood pressure, stomach ulcers, avascular necrosis (a serious joint problem), and infections. Because of all the side-effects, doctors try to wean patients off of steroids as quickly as possible.

Several different tests are currently available and several new tests are being developed; all of these have advantages and disadvantages. The optimal screening test depends on each person's preferences. It is important to review each test's effectiveness, safety, convenience, and cost with your doctor. By working with your doctor, you can design the screening plan that is best for you.

The likelihood that patients with celiac disease will develop other immune-related disorders (such as diabetes mellitus and thyroid disease) may be related to the duration of exposure to gluten.

Ultrasound
The doctor often can diagnose cirrhosis from the patient’s symptoms and from laboratory tests. During a physical exam, for instance, the doctor could notice a change in how your liver feels or how large it is. If the doctor suspects cirrhosis, you will be given blood tests. The purpose of these tests is to find out if liver disease is present. In some cases, other tests that take pictures of the liver are performed such as the computerized axial tomography (CAT) scan, ultrasound, and the radioisotope liver/spleen scan.
MRI
IS THERE ANY WAY I CAN BE SCREENED FOR POLYPS WITHOUT HAVING TO GO THROUGH ANOTHER COLONOSCOPY? — Colonoscopy will remain the best examination for follow-up with patients who have had polyps removed. Although new technologies are being developed that show promise for detecting polyps (including molecular genetic tests and "virtual colonoscopy" using CT or MRI technology), colonoscopy will remain the most efficient means for the foreseeable future.

New tests — Several new screening tests for colorectal cancer are currently being developed and evaluated. These tests include improved fecal occult blood tests, fecal tests for genetic abnormalities linked to colorectal cancer, and a type of computed tomography (CT) scan called a virtual colonoscopy. These tests are still being studied, and their value is not yet well enough defined to be used for routine screening.

WHAT IS THE PROGNOSIS? — The majority of patients completely respond to a gluten-free diet and go on to lead a normal life. Approximately 70 percent of those with symptoms at diagnosis notice an improvement in intestinal symptoms (such as diarrhea) within two weeks of beginning a gluten-free diet; in others, the response is more gradual. The skin lesions present in some patients with celiac disease (dermatitis herpetiformis) may take several months to resolve. The response to some of the other associated conditions is variable.
